精品论文 参考文献 探讨尼可地尔对慢性充血性心力衰竭患者心脏功能的影响 望奎县人民医院 152100 摘要：目的：研究分析尼可地尔对慢性充血性心力衰竭患者心脏功能的影响。方法：此次研究的对象是选择92例慢性充血性心力衰竭患者。将其临床资料进行回顾性分析，并随机分为尼可地尔组和常规治疗组。常规治疗组采用常规抗心衰治疗，尼可地尔组在常规治疗的基础上加用尼可地尔。分别检测两组患者治疗前后左室舒张末期内径（LVEDD）、左室射血分数（LVEF）及左室收缩末期内径（LVESD），测定两组治疗前后6 min步行距离。结果：与常规治疗组治疗后比较，尼可地尔组LVEDD、LVESD明显减小，LVEF和6 min步行距离明显增加（Plt;0.05）。结论：在常规治疗的基础上加用尼可地尔可进一步改善慢性充血性心力衰竭患者的心功能。 关键词：慢性充血性心力衰竭；尼可地尔；6 min步行试验 [Abstract] Objective：To study the effects of nicorandil on cardiac function in patients with chronic congestive heart failure. Methods：the study selected 92 patients with chronic congestive heart failure. The retrospective analysis of the clinical data，were randomly divided into nicorandil group and routine treatment group. The conventional treatment group was treated with conventional anti heart failure treatment，nicorandil group on the basis of conventional treatment combined with nicorandil. The left ventricular end diastolic diameter（LVEDD），left ventricular ejection fraction（LVEF）and left ventricular end systolic diameter（LVESD）were measured before and after treatment in two groups，and 6 min walking distance were measured before and after treatment in two groups. Results：compared with the conventional treatment group after treatment，nicorandil group，LVEDd，LVESD significantly reduced，LVEF and 6 min walking distance increased significantly（P lt; 0.05）. Conclusion：on the basis of conventional therapy plus with nicorandil can further improve the heart function of patients with chronic congestive heart failure. [Key words] chronic congestive heart failure；nicorandil；6 min walking test 慢性充血性心力衰竭是各种原因导致的心功能下降后期患者心功能发生失代偿的表现，是多种心血管系统疾病的最终转归，死亡率高，严重影响人们的身体健康。尼可地尔（nicorandil）是硝酸盐的烟酰胺衍生物，是一种新型钾通道激活剂，具有扩张动静脉、冠脉，增加心肌供血，降低心脏负荷，保护心肌等多重作用，笔者所在医院采用口服尼可地尔治疗慢性充血性心力衰竭患者，观察其临床疗效，现报告如下。 1 资料与方法 1.1 一般资料 将2010年10月-2012年9月笔者所在医院收治的92例慢性充血性心力衰竭患者随机分为尼可地尔组47例和常规治疗组45例。尼可地尔组男31例，女16例；年龄42～69岁，平均（54.7plusmn;3.2）岁；病程1.8～6.2年，平均（4.2plusmn;1.6）年；NYHA心功能分级：Ⅱ级24例，Ⅲ级15例，Ⅳ级8例；
