中医五神之魂魄表征老年性痴呆病机特色的研究 提 要 目的：根据中医“五神”学说，在“五神”内涵与外延的基础上，结合老年性痴 呆认知障碍和精神行为异常的临床特点，在心理层面上从魂魄角度表征老年性痴呆的 病机特色。方法：搜集规模最大历史丛书《四库全书》及中医大型电子图书《中华医 典》中关于“五神”的论述，对其内涵和外延进行诠释和界定，进一步完善和充实“五 神”学说。以“神”、“魂”、“魄”、“意”、“志”为一级检索词，以病理语义词为二级 检索词，共检索出能够表征老年性痴呆症状和体征的词条及描述性词语的词条119 条，借鉴心理测量学的理论和方法，经过文献调研和专家论证，构建出中医五神老年 性痴呆初步调查问卷，通过对临床调查数据的统计分析对条目进行筛选调整，及进一 步大样本调查对量表进行再调整及科学性考核，最后制定出信效度良好的中医五神老 年性痴呆评定量表；最终选取魂魄为切入点，从心理层面探究老年性痴呆的病机特色。 结果：研制出了信、效度较理想的“中医五神老年性痴呆调查表”，能够从中医“五 神”角度更好的诠释老年性痴呆以躯体病变为辅以心理层面为主的病机特色，且从中 抽提魂魄两要素，总结归纳出中医五神之魂魄表征老年性痴呆病机特色：魂魄离守、 魂魄昏蒙、魂魄不足及魂摇魄惊、魂出入失调五个方面，其中“魂魄昏蒙”偏重于“惑” 的表现上，如兴趣度的降低及能力（计算力、工作力）的下降；“魂魄离守”更偏重 于言语及动作行为的异常；“魂魄不足”多表现为睡眠、记忆两方面，睡眠主要强调 白天嗜睡，夜晚失眠，记忆以近期记忆下降为主。“魂出入失调”对睡眠影响较大， 魂不入则失眠，魂不出则嗜睡。 关键词 神；魂；魄；意；志；病机；相关性分析 1 Research on Pathogenesis Characteristics of AD based on Hun Po of the five directional gods of TCM Speciality:Neurology of TCM Author:Wang Xue Tutor:Prof.Teng Jing Abstract Objective: According to the theory of five directional gods of TCM and based on study of senile dementia cognitive impairment and spiritual unusual behavior of the clinical features, we try to have a research on Pathogenesis Characteristics of AD based on Hun Po of the five directional gods of TCM.Methods:Collectthediscussaboutthefivedirectionalgodsfromimperial collection of four and the Medical pawning to enrich the theory of the five gods,through defining its connotation.With god and soul and spirit, meaning and ambition key words for level and with pathological semantic wordsassecondarysearchterm,119entrycanbeusedtocharacterizethesigns and symptoms of dementia and descriptive terms of words article.And then constructUltimatequestionnairethefivegodsandAD,throughScreeningitems pool,analyzing data,expanding the sample size and examining scientifically.Results: The questionnaire possessing good reliability and validitycanwellexplainPathogen
