中国药事 2012年第 26卷第 i2期 对溶血与凝聚检查法的探讨 张 媛，钱德明，刘群丽，高 华 (中国食品药品检定研究院，北京 100050) 摘要 ： 目的 对 《中国药典》2010年版溶血与凝聚检查法的结果判断标准进行初步研究，确定红细胞凝 聚阳性对照物。方法 将一定浓度的高分子右旋糖酐 (Dextran500)或明胶加入到 2 红细胞悬液中，通过 肉眼观察和显微镜观察 2种方法在不 同时间点观察红细胞凝聚的情况，比较 2种观察方法的结果有无差 异，并根据红细胞凝聚情况选择合适的红细胞凝聚阳性对照物及适 宜浓度 。结果 不同浓度的高分子右旋 糖酐和明胶分别与296／兔红细胞悬液混合温育后 ，应用显微镜观察 ，在不同时间点发现红细胞均有不同程 度的聚集，而肉眼观察则未见红细胞凝聚，二者差异明显。其 中4 的高分子右旋糖酐于温育 15min后 ， 即可出现红细胞凝聚，且凝聚程度适中，在较长温育时间内均可保持凝聚状态。结论 在溶血与凝聚检查 法中，以肉眼观察为主的红细胞凝聚实验的结果判断方法存在较大误差，有待 改进。4％的高分子右旋糖 酐可考虑作为红细胞凝聚实验阳性对照物。 关键词 ： 凝聚 ；明胶 ；高分子右旋糖酐 中图分类号 ：R927．12 文献标识码：A 文章编号 ：1002—7777 (2012)12—1340—03 AnExplorationofTestforHaemolysisandAgglom eration ZhangYuan，QianDeming，LiuQunliandGaoHua (NationalInstitutesforFoodandDrugControl， Beijing100050) ABSTRACT： ObjectiveToexplorethejudgmentstandardoferythrocyteagglomeration testinCP (2010)，and determinethepositive controlforagglomeration test．Methods2 erythrocytesuspension wasmixedwithDextran500solutionorgelatin solutionandincubatedin37℃ 。then theagglomeration levelin different time points was observed by both visualinspection and microscope observation． Appropriatepositive controland its concentration foragglomeration testaccording to the erythrocyte agglomerationlevelwaschose．ResultsTheerythrocyteagglomerationwasobservedinmosttimepointsby m icroscope observation， however， none by visualinspection． The 4 Dextran500 induced moderate erythrocyteagglomerationafter15minofincubation，andtheagglomerationlevelW3Sstablymaintained forseveralhours．ConclusionTherearesignificantvariationsin thejudgmentstandardoferythrocyte agglomeration byvisua1inspectionin CP (2010)which needtobeimproved． The4 DextranS00 could berecommendedasthecandidateofpositivecontro1． KEY WORDS： agglomeration；gelatin；DextranS00 溶血与凝聚检查法是观察受试药物是否引起溶 型和Ⅲ型过敏反应 ；非免疫性溶血包括药物为诱发 血和红细胞凝聚反应。药物引起的溶血反应可分为 因素导致的氧化性溶血和药物制剂引起血液稳态的 免疫性溶血和非免疫性溶血两类。免疫性溶血是药
