精品论文 参考文献 颈部血管超声与TCD结合对隐匿型盗血综合征诊断率的提高 湖南省长沙市中心医院超声诊断科 湖南长沙 410004 【摘 要】目的：探讨分析颈部血管超声与TCD结合对隐匿型盗血综合征诊断率的提高。方法：在我院2015年1月1日至2015年8月31日期间脑卒中筛查人群中使用两者相结合的方法共筛选出盗血综合征患者53人，其中经颅多普勒（TCD）检测出的盗血综合征患者50人，而颈部血管超声检测出的盗血综合征患者45人。根据椎动脉的多普勒频谱特征将盗血程度分为3种类型：隐匿性盗血者，表现为收缩期切迹，锁骨下动脉或椎动脉起始段狭窄率约50% ～ 69%；部分性盗血者，表现为收缩期反向血流，舒张期为正向血流或无血流信号，锁骨下动脉或椎动脉起始段狭窄率约70% ～ 99%；完全性盗血者，表现为患侧椎动脉内全心动周期反向血流为完全性盗血锁骨下动脉或椎动脉起始段狭窄率约99%或闭塞。其中漏诊患者多为隐匿型盗血患者，因此，对比发现两者相结合的方法对隐匿型盗血综合征的诊断率高于单独检查其中一项。结论：颈部血管超声与TCD结合对隐匿型盗血综合征的诊断率有明显提高。 【关键词】颈部血管超声；TCD；隐匿型盗血综合征 Abstract Objective：To investigate the improvement of the diagnosis rate of the concealed steal syndrome by the combination of ultrasound and TCD.Methods：in our hospital in January 2015 August 1，to 2015 31 cerebral stroke screening population using both combination method were screened out steal syndrome patients，53，which transcranial Doppler（TCD）detected steal syndrome patients 50 people，and neck vascular ultrasound detection of steal syndrome patients 45 people.According to the Doppler spectrum characteristics of vertebral artery will steal degree is divided into three types：occult steal，systolic notch，the subclavian artery or the initial segment of the vertebral artery stenosis rate of about 50% to 69%；partial steal，reversed flow of systolic diastolic forward flow or no flow signal，subclavian artery or the initial segment of the vertebral artery stenosis rate of about 70% ~ 99%；complete steal，to suffer from side of vertebral arteries throughout the cardiac cycle reverse flow for complete steal blood subclavian artery or the initial segment of the vertebral artery stenosis rate of about 99% or occlusion.Among the patients with occult blood，the diagnosis rate of the concealed type of stolen blood syndrome was higher than that of the patients.Conclusion：the combination of ultrasound and TCD in the diagnosis of occult blood vessel has a significant improvement.
