局限性Castleman 病CT表现 　　[摘要] 目的 分析局限性Castleman 病的CT 表现特征，提高其诊断准确率。 方法 回顾性分析9例具有完整的临床和病理资料的Castleman 病患者的CT表现。 结果 CT 扫描9例均表现为单发软组织肿块，直径为4～9 cm。平扫CT 值为28～46 HU，增强扫描早期显著均匀强化，CT值高达110～187 HU，静脉期CT 值为106～128 HU，延迟期CT值为98～110 HU。病理上包括透明血管型8例和混合型1 例。3 例伴有中央区分支状或点状钙化。 结论 局限性Castleman 病多表现为孤立性软组织肿物，强化明显或中央伴有分支状钙化等特异征象，CT扫描对于该病明确诊断具有重要意义。 　　[关键词] 巨淋巴结增生；体层摄影术；X 线计算机；淋巴结 　　[中图分类号] R445.3 [文献标识码] A [文章编号] 1674-4721（2012）03（a）-0100-02 　　 　　CT features of localized Castleman disease 　　LI Zhongguo CAI Chengshi 　　Department of Radiology, the First Hospital of Huairou, Beijing 101400, China 　　[Abstract] Objective To analysis of the limitations of Castleman disease CT features, to improve the diagnostic accuracy rate. Methods Retrospective analyzed of 9 cases with complete clinical and pathological data of the CT findings of Castleman disease. Results CT scans of 9 cases showed a solitary soft tissue mass, diameter 4-9 cm. Unenhanced CT value 28-46 HU, enhanced scan in early remarkably homogeneous enhancement, high CT value for 110-187 HU, venous phase CT values for 106-128 HU, CT values for 98-110 HU delay period between. Pathology includes hyaline vascular type in 8 cases and 1 case of mixed type. In 3 cases accompanied with central branching or punctate calcification. Conclusion The limitations of Castleman disease manifested as solitary soft tissue masses, enhanced or central with branching calcification of the special signs, CT scanning for the diagnosis of disease that has important significance. 　　[Key words] Giant lymph node hyperplasia; Tomography; X-ray computed; Lymph gland 　　 　　局限性Castleman 病是一种异源性的罕见的淋巴组织增生性疾病，多发于胸部和颈部，发生在其他部位的Castleman 病相对少见。目前，该病CT诊断准确率较低，回顾性分析本院10年来经临床手术病理证实的9例局限性Castleman 病CT 表现，旨在提高对其的认识，提高诊断准确率。 　　1 资料与方法 　　1.1 一般资料 　　搜集2000年1月～2011年8月本院经手术病理证实的局限性Castleman 病9例，其中，胸部4例，颈部2例，腹膜后2例，盆腔1例；男4例，女5例；年龄26～58 岁，平均30岁。临床表现：4例右肺门软组织肿块者表现为咳嗽伴胸闷，少量血丝痰，其余5例均在体检或无意中扪及体表肿块就诊发现。实验室检查：白细胞增高2 例（分别为13.8×109/L 和15.7×109/L），血沉增快6 例（29～54 mm/h），贫血2例（分别为103 g/
