* * * * 气管内吸痰 国外：2010年，修订了吸痰指南。 目的： 途径：经气管插管吸痰 经气管切开吸痰 经鼻腔吸痰（要求润滑） * 吸痰管的要求 理想的吸痰管是经过雾化、毛化的不完全 透明的吸痰管，有一定的硬度。 使用的型号：外径小于气管导管内径的二分之一为宜. 插管与吸痰管相对应的 7.0mm—10FR 7.5mm----12FR 8.0mm----14FR 8.5mm----16FR 不建议太软的硅胶管 * 吸痰的指证 1、咳嗽、憋气。 2、痰鸣 3、起到压力过高或者报警 4、血样饱和度下降 5、频繁呛咳 6、神志清的病人自述 * 吸痰前准备 调节合适的负压吸引成人100—150mmHg，儿童80—100mmHg， 幼儿60—80mmHg. 与患者充分的交流，取得配合。 胸部物理疗法:扣背、湿化。 吸痰前给氧 ：纯氧2分钟。 * 吸痰方法注意事项 打开无菌盐水后，有一只手是绝对无菌的。 无负压情况下，轻轻放入，碰到阻力退后半公分。 插入吸痰管的深度：超过插管1厘米。 旋转手提吸痰管时，同时给负压。 每次吸痰时间小于15秒，注意要固定导管，再吸痰。 * 吸痰方法注意事项 吸痰后，观察氧饱和度。 评估再次吸痰。 实验证明：吸痰很痛苦的事。提拉吸痰两次，就可出血。 返流性误吸：吞咽不好的病人，会厌开放，口腔、气道细菌移行。 去除囊袋上滞留物的方法。 蒸汽加温湿化最好。盐水滴入效果不好， * 吸痰误区 单侧吸痰 痰液湿化不充分 吸痰不彻底 做不到按需吸痰 * 呼吸机管路 国内：要求每周更换一次。 国外：要求每两周已更换。 不同病人使用要更换。 气管插管未证明更换的时间。 * * * * Deflate the cuff of the mask and apply a water-soluble lubricant to the posterior surface. Rub the lubricant over the anterior hard palate (Fig. 1). * Ensure the curved metal tube is in contact with the chin and the mask tip flat against the palate prior to rotation (Fig. 2). * Swing mask into place with a circular motion, maintaining pressure against the palate and posterior pharynx (Fig. 3). * Visually inspect and inflate ETT to verify cuff integrity and symmetry. Deflate cuff, lubricate ETT and pass through LMA Fastrach? tube (rotate with up/down movement) to distribute lubricant. Pass the ETT to 15 cm depth marker or the transverse line on the LMA Fastrach? ETT which corresponds to passage of tube tip through the epiglottic elevating bar (Fig. 1). * Use the handle to gently lift the device 2 - 5 cm as the ETT is advanced (Fig. 2).
