子宫输卵管造影与腹腔镜检查在诊断输卵管性不孕中价值比较 　　【摘要】目的 对比分析子宫输卵管造影与腹腔镜检查在诊断输卵管性不孕中的价值。方法 2009年6月至2011年6月期间在我院就诊的191例不孕症患者行子宫输卵管造影(HSG)检查，所有患者HSG后行腹腔镜诊治。结果 191例患者共374条输卵管中， 经HSG显示输卵管通畅90条，通而不畅76条，近端阻塞74条，远端阻塞134条；经腹腔镜检查显示输卵管通畅104条，通而不畅64条，近端阻塞50条，远端阻塞156条。 HSG与腹腔镜检查输卵管异常对比，差异有显著性。HSG与腹腔镜检查的符合率比较，完全符合率60.4%，其中符合率最高是远端阻塞占83.6%，通而不畅的符合率最低，仅占26.3%，近端阻塞的符合率45.9%，通畅的符合率66.7%。其中 HSG显示通畅90条中，经腹腔镜检查证实其伞端周围与盆腔组织有不同程度粘连或输卵管外形扭曲64条。结论 腹腔镜诊断输卵管不孕具有直观、准确性，同时发现盆腔其它病变情况，并同期得到相应治疗。腹腔镜比子宫输卵管造影有明显优势。 　　【关键词】子宫输卵管造影 腹腔镜 输卵管性不孕 　　中图分类号：R711.6文献标识码：A文章编号：1005-0515（2011）12-001-02 　　【Abstract】Objective To comparate and analysis value of hysterosalpingography(HSG) and laparoscopic examination in the diagnosis of tubal infertility. Method HSG was performed in 191 infertile women treated in our hospital from June 2009-June 2011, laparoscopic examination was carried out in all patients after HSG. Results the HSG’s result of all the 374 fallopian tubes in the 191 cases show : In the total of 374 cases of fallopian tubes with 191 patients,the HSG shows: patency90,passable76, Nearly distal obstruction 74, Distal obstruction 134;the laparoscopic examination shows: patency 104, passable 64, nearly distal obstruction 50, distal obstruction 156. Compared the HSG results of tubal abnormalities with laparoscopic, the difference was significant. The coincidence of HSG with laparoscopic comparison : Full compliance rate60.4%, the highest among them is 83.6% of the distal obstruction, the lowest is 26.3% of the passable, the compliance rate of nearly distal obstruction is45.9%,the compliance rate of distal obstruction is 66.7%.Among the article 90 fallopian tube which HSG shows patency , article 64 were confirmed by laparoscopy , around the end of its umbrella organization with varying degrees of pelvic adhesions or tubal shape distortion. Conclusion The diagnosis of tubal infertility by laparoscope has the advantages of accuracy, we can found that other lesions in the pelvic and get the corresponding treatment at same
