洛赛克静脉注射治疗上消化道出血325例的内镜下观察 司岑 王正 赵晓红 黄睿 张占云 [摘要] 目的 评价静脉注射洛赛克治疗上消化道出血疗效。方法 对325例上消化道出血静脉注射洛赛克，48h内进行内镜检查对出血是否停止进行判定。160例失血超过1000ml，在抗休克治疗中静脉注射洛赛克40mg，2次／d，连用2d，随后20mg／d口服；165例失血少于1000ml，用洛赛克40mg，1次／d，2d改为20mg／d口服。74例肝硬化合并上消化道出血者同时就用善得定0．2ms／d缓慢静脉滴注。结果 洛赛克止血有效率依次为：急性胃粘膜病变100％；肝硬化并上消化道出血98.64％；上消化道癌96．77％；消化性溃疡94．61％；其他原因77．77％。结论 洛赛克静脉注射治疗上消化道出血2d内内镜下完全止血率为96．62％(314／325例)。 [关键词] 静脉注射；洛赛克；上消化道出血；内镜 Vein injection of losec to treat upper gastrointestinal bleeding. SIGen，WANG Zheng，ZHAO Xiaohong，et al．Department of Gastroenterology，Afilliated Hospital of Ning Xia Medical College，Ning Xia，750004，China [Abstract] Objective The efficacy of vein injection of losec as the treatment for parlents with upper gastrointestinal bleeding was assessed．Methods 325 patients with upper gastrointestinal bleeding subjected to the injection， of which 160 severe cases had been injected of losec 40mg，twice a day，after 48 hours they had taken losec 20mg，once a day，165 slight cases had been injected of losec 40mg，once a day， after 48 hours they had taken losec as first group． Endoscopy is the method of establishing whether it is bleding within 2 days．Results It was observed that the bleeding because acute gastfic mucosa lesions was stopped completely， and the effectiveness of stop bleeding are as follow：esophaged varices 98．64％，carcinoma of upper gastrointestinal 96.77％，peptic ulcer 94．61％，others 77．77％，Conclusion Vein injection is an effective and safe alternative for upper gastrointestinal bleeding therapy． [Keywords]Vein injection；Losec；Upper gastrointestinal bleeding；Endoscopy 上消化道出血是临床常见的急症，常采用各种方法治疗，以粪潜血转阴为指标判定出血 停止时间。我院自1995年采用洛赛克(阿斯利康公司产品)40—80mg／d静脉注射治疗上消化道出血325例，于用药48h行内镜检查，采用Forrest法，观察止血效果，现报告如下。 资料与方法 一、一般资料 1995年1月—1999年6月以上消化道出血收住的患者325例，男265例，女60例，年龄为13—90岁。失血量为1500ml—2000ml者11例，1000ml—1500者149例，小于1000ml者165例。 二、治疗方法 常规给予输血、补充血容量、抗休克综合治疗，均不应用其他止血药。失血超过1000ml者 静脉注射洛赛克40mg，2次／d，失血小于1000ml看静脉汪射浴赛克40mg，1次／d，连用2d，随后20mg／d口服。肝硬化并上消化道出血患者，同时应用善宁0．2mg／d静脉
