三维CTA超选择性动脉栓塞治疗植入性胎盘临床疗效分析 　　【摘要】 目的：探讨三维CTA超选择性动脉栓塞治疗植入性胎盘的临床疗效，并对其分析。方法：60例胎盘植入患者随机分为试验组和对照组。试验组在三维CTA辅助下行超选择植入性胎盘的供血动脉（子宫动脉卵巢支或者输卵管支）造影+栓塞，对照组行传统的子宫动脉造影+栓塞，栓塞的均不是既往的子宫动脉。检测两组出血、血压、胎盘组织排出、子宫复旧和血绒毛膜促性腺激素β亚单位（β-hCG）水平的差异。结果：试验组的各项临床检测指标与对照组相比较，差异有统计学意义（P　　【关键词】 三维CTA； 超选择性； 动脉栓塞； 植入性胎盘 　　中图分类号 R714.46 文献标识码 A 文章编号 1674-6805（2014）24-0005-04 　　The Clinical Curative Effect of 3D CTA Super Selective Arterial Embolization in the Treatment of Implanted Placenta/QIN Yi-wei，ZHANG Xiao-li.//Chinese and Foreign Medical Research，2014，12（24）：5-8 　　【Abstract】 Objective：To study clinical curative effect of the 3D CTA super selective arterial embolization in the treatment of implanted placenta，and analyse it.Method：60 cases of implanted placenta were randomly divided into the experimental group and control group.The experimental group in the 3D CTA auxiliary downward choosed implanting placental blood supply artery（uterine artery ovarian or tubal branch） to angiography plus embolization，the control group was treated with traditional uterus arteriography plus embolization.Uterine artery embolization was not before.To detect bleeding，blood pressure，placental tissue eduction，uterine instauration and blood chorionic gonadotropin beta subunits（beta hCG） level difference in two groups.Result：The clinical detection indicators of the experimental group compared with control group，there were statistical significance（P0.05），见表1。 　　表1 两组患者术前资料数据结果比较 　　组别 β-hCG 　　（U/L） 累计出血量（ml） 术前收缩压（mm Hg） 术前舒张压（mm Hg） 　　试验组（n=30） 3660±285 1530±340 98±13 63±17 　　对照组（n=30） 3592±297 1670±261 92±14 59±15 　　1.2 方法 　　1.2.1 介入治疗术前准备 前常规予促宫缩、输血输液补充血容量和纠正休克治疗，维持生命体征平稳。常规检查血绒毛膜促性腺激素（β-hCG）[3]。 　　1.2.2 介入方法 对照组：使用V 3000型血管造影机（荷兰Philips公司生产）行子宫动脉造影，采用改良式Seldinger技术，经右股动脉途径穿刺置管，用4.0F或5.0F子宫动脉导管行两侧髂内动脉插管，DSA检查，显示出血动脉及子宫动脉的开口及走行，选择子宫动脉插管。之后用500～700 μm的明胶海绵颗粒（杭州艾力康医药科技有限公司生产）+造影剂（碘普罗胺0.796 g/ml）混合，在X线透视下栓塞双侧子宫动脉，以造影剂停滞无反流，且造影复查显示子宫异常染色消失、保留子宫动脉主干为完全栓塞的标准。经髂内动脉造影进一步证实栓塞成功后取出导管、导管鞘，穿刺点压迫止血，局部加压包扎[4]。 　　试验组：（1）西门子SOMATOM
