4例多形性脂肪瘤针吸细胞学诊断体会 　　[摘要] 目的 探讨针吸细胞学对多形性脂肪瘤的诊断要点与价值。 方法 回顾4例多形性脂肪瘤的FNAC资料及相关临床表现，并复习相关文献，分析总结FNAC诊断要点。 结果 多形性脂肪瘤的细胞形态学具有欺骗性，无诊断经验者易误诊为软组织恶性肿瘤。但掌握其细胞学特点后可报告：细胞形态符合多形性脂肪瘤表现。 结论结合临床表现，根据细胞学特征，尤其是花环状多核巨细胞的出现，FNAC可对多形性脂肪瘤作出明确的符合性诊断。 　　[关键词] 多形性脂肪瘤；针吸细胞学；花环状多核巨细胞；诊断体会 　　[中图分类号] R738.6 [文献标识码] B [文章编号] 1673-9701（2015）12-0087-03 　　Diagnosis experience of fine needle aspiration cytology of 4 cases with pleomorphic lipoma 　　ZHU Siguo1 YU Bo2 　　1.Department of Clinical Laboratory， the Peoples Hospital of Jianli County in Hubei Province， Jianli 433300， China； 2.Department of Clinical Laboratory， the First Peoples Hospital of Xiangyang City in Hubei Province， Xiangyang 441000， China 　　[Abstract] Objective To explore the diagnostic criteria and clinical value of fine needle aspiration cytology for pleomorphic lipoma. Methods The data of the FNAC and related clinical manifestations of 4 cases of pleomorphic lipoma were reviewed and literatures on FNAC were observed， analyzed the diagnostic criteria of FNAC in cytological features. Results Cytological features of pleomorphic lipoma were deceptive， and were easy to be misdiagnosed as a mali-gnant soft tissue tumor by no diagnostic experience. Cytological features were conformed with pleomorphic lipoma’s manifestations that could be reported through mastering the cytological features. Conclusion According to the cytological features， combined with clinical manifestations， especially the appearance of multinucleated giant cells， FNAC can make a definite diagnosis of pleomorphic lipoma. 　　[Key words] Pleomorphic lipoma； Fine needle aspiration cytology； Rosette shapedmultinuclear giant cells； Diagnosis experience 　　多形性脂肪瘤（pleomorphic lipoma，PL）是一种少见的皮下软组织良性肿瘤，但是其细胞形态具有不同程度的异型性，如果缺乏诊断经验，极易被其所欺骗而误诊为软组织恶性肿瘤。本文通过回顾性分析4例PL的针吸细胞学（fine needle aspiration cytology，FNAC）特征，并复习相关研究资料，旨在探讨FNAC对PL的诊断要点与价值。现报道如下。 　　1 资料与方法 　　1.1 一般资料 　　2002年1月～2012年10年间共有4例患者行FNAC检查后，经组织病理学诊断为PL，并此4例患者至今都健在，临床随访3～10年，未见恶性肿瘤复发与转移。其中监利县人民医院3例，襄阳市第一人民医院1例。回顾
