低分子肝素治疗原发性肾病综合症的临床疗效观察 　　摘要：目的 观察低分子肝素治疗原发性肾病综合症的临床效果。方法 随机选择2015年1月～2016年5月我院肾病科76例肾病综合症患者，随机分为两组，对照组采用强的松等常规治疗，治疗组在强的松等常规治疗的基础上加用低分子肝素，两组治疗前后分别检测24 h尿蛋白、血浆白蛋白、总胆固醇、血纤维蛋白原、凝血酶原时间、部分凝血酶时间、血小板计数，并观察临床症状改善情况及有无出血倾向。结果 治疗4 w后，与对照组相比较，治疗组24 h尿蛋白定量明显降低，血浆白蛋白明显升高，血浆纤维蛋白原明显降低（P0.05）。结论 低分子肝素能减少原发性肾病综合症患者尿蛋白，升高血浆白蛋白，缓解高凝状态，预防血栓形成，并有很好的安全性。 　　关键词：低分子肝素；原发性肾病综合症；24 h尿蛋白；血纤维蛋白原 　　中图分类号：R692 文献标识码：A 文章编号：1006-1959（2017）17-0047-03 Clinical Observation of Low Molecular Weight Heparin in the Treatment of Primary Nephrotic Syndrome 　　WANG Li 　　（Department of Nephrology，Lianyungang Chinese Medicine Hospital，Nanjing University of Traditional Chinese Medicine，Lianyungang 222000，Jiangsu，China） 　　Abstract：Objective To observe the clinical effect of low molecular weight heparin in the treatment of primary nephrotic syndrome.Methods Randomly selected January 2015 to May 2016，in our hospital department of nephropathy，76 cases of nephrotic syndrome were randomly divided into two groups，the control group was given prednisone and other conventional treatment，the treatment group with low molecular weight heparin on the basis of prednisone and other conventional treatment，24h urinary protein，serum albumin，total cholesterol，fibrinogen，prothrombin time，partial thromboplastin time and platelet count were detected before and after treatment in two groups，and observe the improvement of clinical symptoms and bleeding tendency.Results After 4 w treatment，compared with the control group，the treatment group 24 h urinary protein significantly decreased，serum albumin significantly increased，plasma fibrinogen decreased significantly（P0.05）.Conclusion Low molecular weight heparin can reduce primary nephrotic syndrome patients with urinary protein，increase plasma albumin，alleviate hypercoagulable state.The prevention of thrombosis，and has good security. 　　Key words：Low molecular weight heparin；Primary nephrotic syndrome；24 h urine protein；Fibrinogen 　　肾变病综合征是指由多种病因引起的，以肾小球基膜通透性增加伴肾小球滤过率降低等肾小球病变为主的一组
