不同程度冠状动脉狭窄患者应用64层螺旋CT诊断的临床分析 　　【摘要】 目的 分析不同程度冠状动脉狭窄患者应用64层螺旋CT诊断的临床价值。方法 选取60例疑似冠心病患者作为研究对象，分别采用64层螺旋CT和常规冠状动脉造影进行诊断， 观察64层螺旋CT诊断结果的正确率、特异性、敏感度、阳性预测值、阴性预测值。结果 在轻度狭窄程度的诊断上， 64层螺旋CT诊断漏诊4个；在中度狭窄程度的诊断上， 64层螺旋CT诊断漏诊7个；在重度狭窄程度的诊断上， 64层螺旋CT诊断多诊2个。64层螺旋CT在诊断轻度狭窄的敏感性为85.7%、特异性为96.7%、阳性预测值为89.1%、阴性预测值为94.8%；在诊断中度狭窄的敏感性为87.4%、特异性为97.8%、阳性预测值为91.8%、阴性预测值为97.8%；在诊断重度狭窄的敏感性为96.9%、特异性为99.0%、阳性预测值为94.1%、阴性预测值为96.9%。结论 应用 64层螺旋CT诊断不同程度冠状动脉狭窄患者， 其诊断正确率、特异性、敏感度及阳性阴性预测值都非常高， 具有重要的临床价值。 　　【关键词】 冠状动脉狭窄；64层螺旋CT；临床价值 　　【Abstract】 Objective To analyze the clinical value of 64-slice spiral CT applied in diagnose of patients with different degrees of coronary stenosis. Methods A total of 60 suspected coronary heart disease patients as study subjects respectively received 64-slice spiral CT and conventional coronary arteriography for diagnosis. Observation was made on diagnosis accuracy rate， specificity， sensibility， positive predictive value and negative predictive value by 64-slice spiral CT. Results 64-slice spiral CT had 4 missed diagnosis in diagnosis of mild stenosis degree， 7 missed diagnosis in diagnosis of moderate stenosis degree， and 2 additional diagnosis in diagnosis of severe stenosis degree. 64-slice spiral CT had sensibility as 85.7%， specificity as 96.7%， positive predictive value as 89.1%， negative predictive value as 94.8% in diagnosis of mild stenosis， which were 87.4%， 97.8%， 91.8%， 97.8% in diagnosis of moderate stenosis， and 96.9%， 99.0%， 94.1%， 96.9% in diagnosis of severe stenosis.
