三种方法消除乳糜血干扰血细胞分析的应用 　　【摘要】 目的：研究消除乳糜血干扰库尔特LH750血液分析仪主要检测指标的方法。方法：选取每日仪器检测后已明确的乳糜血标本，并在2 h内随机选择稀释、空白、置换三种方法进行再检测分析，主要分析WBC、RBC、PLT、Hb、HCT、MCV、MCH、MCHC、NEU%、LYM%、MON%、EOS%、BASO%共13个参数指标，累计检测标本共95份。所有实验标本剩余血液经生化仪检测甘油三酯含量，并以TG0.05）。轻中度乳糜组仅稀释法处理前后的Hb、MCH比较差异有统计学意义（P0.05）；但重度乳糜组Hb、MCH、MCHC、PLT、EOS%三种方法处理前后比较，差异均有统计学意义（P0.05）。结论：乳糜血干扰血细胞分析的主要参数指标为Hb、MCH、MCHC、PLT，影响大小与乳糜血严重程度有关，稀释、空白、置换三种方法均能消除这种干扰，但以稀释法操作方便、可检出较小的干扰，具有推广价值。 　　【关键词】 乳糜血； 血液分析仪； 标本 　　中图分类号 R446.1 文献标识码 B 文章编号 1674-6805（2016）24-0043-04 　　【Abstract】 Objective：To investigate the method for eliminating interference of chylemiaon on the main indexes of COULTER LH750 hematology analyzer.Method：Chyle blood samples which had been cleared by daily testing instruments was selected，they were detected and analyzed by one of the three methods of dilution，blank，replacement randomly within 2 hours mainly to analyze the 13 parameters of RBC，WBC，PLT，Hb，HCT，MCV，MCH，MCHC，NEU%，LYM%，MON%，EOS%，BASO%，a total of 95 samples were detected.The content of triglyceride of all experimental remaining specimens were tested by blood biochemical analyzer，specimens with TG0.05）.The differences in Hb，MCH of chylous mild to moderate group before and after processing by dilution method were statistically significant（P0.05）.But in the chylous severe group，the Hb，MCH，MCHC，PLT，EOS% before and after processing by the three kinds of methods were statistically significant（P0.05）.Conclusion：Chylous disturbs the blood cell analysis of Hb，MCH，MCHC，PLT，the influence degree is associated with the severity of chylemia.The three methods of dilution，blank，replacement can eliminate the interference，but the method of dilution is easy to operate and can detect small interference，it has the value of popularization. 　　【Key words】 Chylemia； Hematology analyzer； Specimen 　　First-author’s address：Maternal and Child Health Hospital of Haiyan County，Haiyan 314300，China 　　doi：10.14033/j.cnki.cfmr.2016.24.024 　　库尔特LH750是贝克曼库尔特公司一款比较经典的五分类血液分析仪，仪器工作原理为细胞计数采用阻抗法、
