三联手术疗法治疗急性嵌顿环状混合痔的疗效观察 　　[摘要] 目的: 探讨肛门内括约肌切断、分段齿状外剥内扎、肛门皮瓣整形三联手术治疗急性嵌顿环状混合痔的临床疗效。方法：将急性嵌顿环状混合痔患者80例随机分为治疗组和对照组各40例，治疗组采用三联手术治疗，对照组采用传统的外剥内扎术治疗，并对二组患者的术后疼痛、肛缘水肿、肛缘愈合时间、肛门狭窄、术后尿潴留进行观察比较。结果：治疗组疗效明显优于对照组（P0.01），治疗组伤口愈合时间，术后疼痛，肛缘水肿，肛门狭窄，尿潴留发生率明显低于对照组（P0.01）。结论：采用肛门内括约肌切断、分段齿状外剥内扎、肛门皮瓣整形三联手术治疗急性嵌顿环状混合痔能迅速减轻患者痛苦，创面恢复快，无明显后遗症及并发症，疗效确切。 　　[关键词] 痔；三联手术；疗效 　　[中图分类号] R657.1+8 [文献标识码] B [文章编号] 1673-7210（2012）01（a）-156-02 　　 　　Observation of curative effect of the triple therapy in the treatment of the circumference mixed hemorrhoids with acute incarcerated 　　TANG Guohui, WU Zhousong, DENG Qiulin, LIU Libing 　　Department of Colorectal Surgery, the Nanhua Hospital Affiliated to Nanhua University, Hu′nan Province, Hengyang 421002, China 　　[Abstract] Objective: To evaluate the curative effect of the partial incision of internal sphincter plus external dissection and internal ligation and plastic the flap of the anus of the triple therapy in the treatment of the circumference mixed hemorrhoids with acute incarcerated. Methods: 80 patients with the circumference mixed hemorrhoids with acute incarcerated were randomly divided into treatment group and control group (40 cases, each group), which both groups respectively received the triple therapy, routine external dissection and internal ligation, then their analgesic effect, edema of anal edge, wound healing time, urinary retention, anal stricture were compared after the operation. Results: The treatment group could improve the analgesic effect, shorten the wound healing time and decrease edema of anal edge, urinary retention and anal stricture rate of post-operative complications. Its effect was better than that of control group (P0.01). Conclusion: The triple therapy can improve the analgesic effect, shorten the wound healing time and decrease post-operative complications.
