乳腺癌前哨淋巴结活检替代腋淋巴结清扫的临床应用研究 　　[摘要] 目的 探讨在乳腺癌临床治疗中采用前哨淋巴结活检替代腋淋巴结清扫的应用价值。 方法 选择2015年1月～2016年1月在我院接受治疗的女性乳腺癌患者100例，随机平均分为研究组和对照组，每组50例。研究组患者行前哨淋巴结活检，对照组患者开展乳腺癌改良根治术或保乳术，对比两组并发症率、平均住院时间及住院费用，统计治疗前后两侧上臂臂围差、肩关节功能评分、肩关节外展角度的变化情况。 结果 研究组患者并发症发生率、平均住院时间（9.14±1.37）d、住院费用（52113.36±137.67）元、治疗后上臂臂围差（0.26±0.05）cm、肩关节功能评分（89.42±3.56）分、肩关节外展角度（142.61±9.57）°，均显著优于对照组，差异有统计学意义（P0.05）。 结论 乳腺癌前哨淋巴结活检替代腋淋巴结清扫能有效减少并发症，缩短住院时间。 　　[关键词] 乳腺癌；前哨淋巴结活检；腋淋巴结清扫；改良根治术 　　[中图分类号] R737.9 [文献标识码] B [文章编号] 1673-9701（2016）14-0053-03 　　[Abstract] Objective To discuss the application value of sentinel lymph node biopsy（SLNB） instead of axillary lymph node dissection （ALND） in breast cancer. Methods 100 patients with breast cancer treated in our hospital from January 2015 to January 2016 were selected and randomly and equally divided into study group and control group（n=50）. Patients in the study group were given SLNB， which those in the control group were given modified radical mastectomy or breast conserving surgery. The incidence rates of complications， hospital stays and costs were compared， and the difference value of upper arm circumference， function scores of shoulder joint， and angle changes of shoulder abduction before and after treatment were observed. Results The incidence of complications， average hospital stay[（9.14±1.37） d]， hospitalization cost [（52113.36±137.67） yuan]， difference value of upper arm circumference after treatment [（0.26±0.05） cm]， function scores of shoulder joint （89.42±3.56）， and shoulder abduction angle （142.61±9.57）° in the study group were all significantly superior to those in the control group （P0.05）.
