临床药师参与心脏机械瓣膜置换术后华法林抗凝治疗管理的方法及作用分析 　　摘要：目的 本文就临床药师参与心脏机械瓣膜置换术后华法林抗凝治疗管理的方法及作用进行分析与探讨。方法 选择我院2014年2月～2016年2月收治的行心脏机械瓣膜置换术患者66例，其后将本组患者按照来院先后顺序平均分为两组，每组患者33例。予以临床药师参与华法林抗凝治疗管理的患者设为实验组，未经临床药师参与的患者设为参照组，最后比对两组患者的抗凝效果。结果 通过对两组患者进行分析可知，实验组患者的INR达标率和出院前INR达标率较比参照组明显较高，INR首次达标时间较比参照组明显较短，出血率同参照组进行比对明显较低，组间统计后差异显著。结论 患者行心脏机械瓣膜置换术时，临床药师参与抗凝管理，其效果理想，具有临床应用价值。 　　关键词：临床药师参与；心脏机械瓣膜置换术；华法林抗凝管理；作用分析 　　中图分类号：R654.2 文献标识码： A 文章编号：1006-1959（2017）13-0079-02 　　Abstract：Objective To study the clinical pharmacists involved in mechanical heart valve replacement after the analysis and discussion of the method of warfarin anticoagulation management and effect.Methods 66 for mechanical heart valve replacement patients in our hospital in February 2014～ February 2016 were followed in this group of patients to the hospital in accordance with the order of the average divided into two groups，33 cases in each group be patient.Clinical pharmacists in warfarin anticoagulation management of patients for the experimental group，control group without the participation of clinical pharmacists in the patients，finally on anticoagulant effect of the two groups.Results Through the analysis of two groups of patients，patients in the experimental group the success rate of INR and INR before discharge compliance rate was significantly higher than the control group，INR for the first time，the standard time is significantly shorter than the control group，the bleeding rate compared with the reference group were significantly lower，the differences between the two groups after statistical significant.Conclusion When the patients underwent mechanical heart valve replacement，the clinical pharmacists participated in anticoagulation management，and the effect was ideal，and it has clinical value. 　　Key words：Clinical pharmacist participation；Cardiac mechanical valve replacement；Warfarin anticoagulant management；Role analysis 　　在心脏病疾病中，最为常见的为风湿性心脏病，该疾病会对心脏瓣膜造成影响，同时致使瓣膜功能出现异常[1]。从目前的治疗方法来看，临床常采用瓣膜修复法和人工瓣膜置换手术，虽然上述治疗手段可以使
