医院―社区一体化管理模式对老年2型糖尿病患者血糖控制影响 　　[摘要]目的 探?对老年2型糖尿病患者实施医院-社区一体化管理模式对患者血糖控制效果的影响。方法 选取本社区健康服务中心2015年9月～2016年9月收治的120例老年2型糖尿病患者作为研究对象，按照随机数字表法分为传统组（n=60）和一体组（n=60），传统组患者进行传统院内管理，一体组患者实施医院-社区一体化管理模式，比较两组的血糖控制效果、糖尿病知识认知评分、治疗依从性状况。结果 一体组患者管理1年后空腹血糖、餐后2 h血糖水平低于传统组，差异有统计学意义（P0.05）。一体组患者管理后的糖尿病知识认知评分、治疗依从性高于传统组，差异有统计学意义（P0.05）。结论 对老年2型糖尿病患者实施医院-社区一体化管理模式可提高患者的血糖控制效果。 　　[关键词]老年2型糖尿病；医院-社区一体化管理模式；血糖控制效果；治疗依从性 　　[中图分类号] R197.4 [文献标识码] A [文章编号] 1674-4721（2018）4（c）-0140-03 　　Effect of Hospital-Community Integrated Management on blood glucose control in elderly patients with type 2 diabetes 　　WAN Lei DONG Kun 　　The community health service center of Southern University of Science and Technology，Nanshan District People′s Hospital of Shenzhen City in Guangdong Province，Shenzhen 518100，China 　　[Abstract]Objective To investigate the effect of hospital-community integrated management on blood glucose control in elderly patients with type 2 diabetes.Methods A total of 120 elderly patients with type 2 diabetes from September 2015 to September 2016 treated in our community health service center were selected as study subjects，and divided into the traditional group （n=60） and the integrated group （n=60） according to the random number table method.The patients in traditional group were treated by traditional hospital management.The patients in integrated group were treated with hospital-community integrated management mode.The blood glucose control effect，cognitive knowledge of diabetes knowledge and treatment compliance were compared between the two groups.Results After one year of management，the fasting blood glucose of patients in integrated group was lower than that of the traditional group at 2 h after meal.The difference was statistically significant （P0.05）.Cognitive score of diabetes knowledge and treatment compliance in the integrated group were higher than those in the traditional group （P0.05）.Conclusion The hospital-community integrated management model for elderly type 2
