处方点评在规范门急诊处方工作中应用效果 　　[摘要] 目的 探讨处方点评在规范门、急诊处方工作中的应用效果。 方法 抽取我院2013年1～12月和2014年1～12月门、急诊处方各1800张，逐一审查，填写处方点评工作表，根据《处方管理办法》和有关文献对处方书写规范性、用药合理性进行评价，对其结果进行统计分析。 结果 实施处方点评后，处方合格率由68.4%提高到89.2%，2013、2014年不规范处方率分别为20.4%、6.0%，用药不适宜处方率分别为11.2%、4.8%，不规范处方（处方修改、药师签名除外）和用药不适宜处方数差异有统计学意义（P0.05）。 结论 实施处方点评和通过采取多种干预措施提高了我院门、急诊处方合格率和合理用药水平。 　　[关键词] 门、急诊处方；处方点评；应用效果 　　[中图分类号] R952 [文献标识码] A [文章编号] 1674-4721（2016）03（b）-0169-03 　　[Abstract] Objective To discuss the application effect of prescription comment in standardizing prescription from outpatient and emergency department. Methods Each 1800 prescriptions from outpatient and emergency department was extracted in our hospital from January to December 2013 and from January to December 2014.They were reviewed one by one and prescription comment sheet was filled in.Based on Prescription management measures and related literature，the normalization and rationality of drug usage was evaluated respectively and the outcomes were statistically analyzed. Results After carrying out prescription comment，the qualified rate was increased from 68.4% to 89.2%.The non-normalization rate of prescription in 2013 and 2014 was 20.4% and 6.0% respectively，and inappropriate rate of drug usage was 11.2% and 4.8% in turn，and there was a statistical difference of number of non-normalization prescription （apart from prescription modification and pharmacist signature） and inappropriate prescription （P0.05）. Conclusion Implementation of prescription comment and other intervention measures increases the qualified rate of prescription and the level of rational drug usage in outpatient and emergency department of our hospital. 　　[Key words] Prescription from outpatient and emergency department；Prescription comment；Application effect 　　为了规范医师处方行为，提高处方质量，保障医疗安全，2010年原卫生部发布了《医院处方点评管理规范》（下称《规范》）[1]，其中详细规定了处方点评内容。2014年我院处方点评小组每月抽取150张门、急诊处方进行点评，统计分析处方点评前后的处方情况，并对处方书写不规范及不合理用药现象，我院医务科采取多种干预措施，旨在提高我院门、急诊处方合格率和合理用药水平。 　　1 资料与方法 　　1.1 一般资料 　　资料来源于2013年1～12月（点评前）
