壮医药线点灸对偏头痛患者血浆 5-HT 的影响 中 文 摘 要 目 的 ：观 察 壮 医 药 线 点 灸 治 疗 偏 头 痛 的 临 床 疗 效 及 患 者 血 浆 五 羟 色 胺 （ 5 - HT ） 指 标 的 变 化 并 探 讨 其 作 用 机 制 。 方 法 ：选取 60 例偏头痛患者，按数字表随机法将其分成治疗组与对 照组各 30 例，治疗组采用壮医药线点灸疗法，以患侧风池、率谷、太阳为 主穴，每穴点灸 3 壮，每日 1 次。30 天为一个疗程。对照组口服西比灵（盐 酸氟桂利嗪）胶囊，每日一次，每次 5mg，睡前服，30 天为一个疗程。采 用自身对照及组间对照两种方法，治疗 30 天后评定疗效。观察两组治疗前 后偏头痛的发作次数、头痛程度、持续时间及伴随症状的改变，根据 VAS 法偏头痛程度计分、偏头痛症状积分对比观察疗效,并观察治疗前后血浆 5- 羟色胺浓度。 结果：（1）治疗组与对照组的组内比较中，治疗前后患者血浆 5-HT 浓度比较（P＜0.01），差异有显著统计学意义；两组治疗后血浆 5-HT 浓度 比治疗前增高（P＜0.05），差异有统计学意义。（2）两组治疗前后的 VAS 法头痛计分观察比较（P＜0.01），差异有显著统计学意义，两组治疗后 VAS 法头痛计分比较（P＜0.05），差异有统计学意义。（3）两组在治疗前后头 痛程度、发作次数、持续时间和伴随症状比较（P＜0.01），均具有极显著 性差异；两组在降低头痛发作次数和头痛持续时间、减少伴随症状方面（P ＜0.05），均有显著性差异，在比较减轻头痛程度方面（P＜0.01），具有极 显著性差异，说明治疗组疗效优于对照组。（4）两组在治疗前后综合计分 比较，具有极显著性差异（P＜0.01），治疗后两组的偏头痛综合计分比较 （P＜0.05），具有显著性差异。（5）两组患者经治疗后，治疗组治愈率 16.67%，总有效率为 90.0%；对照组治愈率 3.33%，总有效率为 80.0%（P ＜0.05），说明治疗组的作用优于对照组。 结 论 ：壮医药线点灸疗法可以有效改善偏头痛症状,其疗效优于口服 西比灵胶囊的疗效，且其安全、无副作用。其作用机制可能与提高患者血 浆中 5-HT 含量有关。 关键词： 偏头痛，壮医药线点灸，5-HT The Study on the Relationship and Influence of 5-HT and Clinical Efficacy on Migraine Treated by Moxibustion of Zhuang Medicine ABSTRACT Objective: To observe the clinical efficacy of migraine and the change of 5-HT which treated by moxibustion of Zhuang medicine. And explore its mechanism. Methods: 60 patients are divided into treatment and control group randomly. 30 people are in each group. Treatment group is treated by moxibustion of Zhuang medicine. The main points include Fengchi(GB20), Shuaigu(GB8) and Taiyang(EX-HN5). Modify points according to the symptoms. We cauterize 3 times every point, once a day. The patients who are in control group take oral Flunarizine Hydrochloride Capsules before sleeping every night, 5mg one time. 30 times is a course.
