皮下埋植避孕剂对使用者体重影响 　　【摘要】目的：探讨皮下埋植避孕剂对使用者体重的影响以及可能的作用机制。方法：通过检索相关文献，了解皮下埋植避孕剂使用者的体重变化及其可能的作用机制。结果：（1）短期放置皮下埋植避孕剂（12个月以内）对使用者的体重影响不明显；（2）长期放置皮下埋植避孕剂（12月以后）对使用者的体重影响较为明显；（3）皮下埋植避孕剂导致体重变化的机制与瘦素有关，但是具体作用机制不详。结论：长期使用皮下埋植避孕剂对使用者体重有影响。 　　【关键词】皮下埋植避孕剂；体重；瘦素 　　Body weight variation in females with subdermal contraceptive implantXU Yumei， TAO Guozhen， SONG Yan，YU Ling， WU Ximei， YANG Yan， SONG Xia. Short-term subdermal contraceptive implant did not significantly affect the users’ body weight. 2. Long-term （over 12 months） subdermal contraceptive implant did significantly affect the users’ body weight. 3. The mechanism of subdermal contraceptive implant leading to the users’ body weight changes was associated with leptin， but the specific mechanism was still unknown. Conclusion： The users’ body weight can be affected by the use of long-term subdermal contraceptive implant. 　　【Key words】Subdermal contraceptive implant； Body weight； Leptin 　　【中图分类号】R169【文献标志码】A 　　皮下埋植避孕法是一种新型、高效、长效、可逆性强的避孕方法。皮下埋植避孕剂是一种孕激素类避孕药，埋植于育龄妇女皮下，药物以缓慢恒定的速度释放进入血液，达到避孕效果。皮下埋植避孕剂主要成分是左炔诺孕酮（LNG），为全合成的强效孕激素，主要作用于下丘脑和垂体，使月经中期FSH和LH水平的高峰明显降低或消失，抑制卵巢排卵，有明显的抗雌激素活性，能使宫颈黏液变稠阻碍精子穿透；左炔诺孕酮对子宫内膜转化显示极强的孕激素活性，可使子宫内膜变薄，内膜上皮细胞呈低柱形，分泌功能不良，不利于孕卵着床，从而起到避孕作用。 　　我国推广应用的皮下埋植避孕剂为左炔诺孕酮硅胶棒埋植剂Ⅰ型（6根），每根含LNG36mg，总量216mg；炔诺孕酮硅胶棒埋植剂Ⅱ型（2根），每根含LNG75mg，总量150mg。国外引进的NorplantⅠ型（6根），每根含LNG36mg，总量216mg；Ⅱ型（2根），每根含LNG70mg，总量140mg[1]。目前引进的进口皮下埋植避孕剂为依托孕烯（1根），每支含依托孕烯68mg。依托孕烯是新一代强效孕激素，是去氧孕烯衍生物。 　　皮下埋植避孕剂常见的
