胰胆管系统先天性畸形MRCP成像中应用价值 　　【摘要】 目的：评价磁共振胰胆管水成像（MRCP）技术在胰、胆管先天性疾病及解剖变异中的临床诊断价值。方法：回顾性分析千余例MRCP检查临床怀疑胰胆管病变，去除各种胆道肿瘤、结石、炎性病变、术后复查及MRCP患者呼吸配合不佳导致图像质量下降外，符合本组研究测量和诊断要求者共158例，对MRCP MIP像结合T2WI压脂横轴位（MRCP定位相）、MRCP原始薄层冠状位断层像综合分析诊断。结果：MRCP对胰胆管各种先天性疾病及其他解剖变异的显示率100%。结论：各种先天性胰胆管疾病及解剖变异的MRCP图像均有特征性表达，MRCP能清晰、直观、立体地显示胰、胆管的解剖结构，有利于胰胆管的整体显示，对胆道系统各种先天性疾病具有重要的临床应用价值。 　　【关键词】 磁共振胰胆管水成像； 胰胆管系统； 先天性畸形 　　【Abstract】 Objective：To evaluate the clinical value of magnetic resonance cholangiopancreatography（MRCP）in the diagnosis of congenital diseases of pancreas and bile duct and anatomic variation.Method：A retrospective analysis of more than one thousand cases of MRCP clinical suspected pancreaticobiliary duct lesions，removed all kinds of biliary tumors，stones，inflammatory lesions，postoperative review and MRCP patients respiratory poor results in the quality of the image decreased.There were 158 patients who met the requirement of measurement and diagnosis in this group.The MRCP MIP images were combined with T2WI.Comprehensive analysis and diagnosis of MRCP original thin layer coronal tomography.Result：The display rate of MRCP for congenital diseases and other anatomical variations of pancreaticobiliary duct 100%.Conclusion：The MRCP images of various congenital cholangiopancreatic diseases and anatomic variations have characteristic expression，MRCP can clearly，intuitively and stereoscopic display the anatomical structure of the pancreas and bile duct，and it is beneficial to the whole display of pancreaticobiliary duct.It has important clinical application value for various congenital diseases of biliary system. 　　【Key words】 MRCP； The pancreatic duct system； Congenital malformation 　　First-author’s address：The First People’s Hospital of Jinzhou，Dalian 116100，China 　　doi：10.3969/j.issn.1674-4985.2018.04.030 　　胰?管系统先天性疾病及解剖变异临床并不少见，了解胆道系统变异的解剖规律，有助于提高胆道系统各种相关疾病的诊治水平。近年来有关胰胆管汇合异常与胰胆系疾病之间的关系的研究备受关注，尤其日本学者的研究比较深入，但所报道的主要是通过尸体解剖和内窥镜逆行胰胆管造影术（ERCP）对胰胆管解剖学特点及分型的研究[1]。磁共振胰胆管成像（magnetic resonance c
