阴道分泌物五联检试剂盒临床应用评价 　　摘要：目的 评价硕世阴道炎五联检试剂盒的临床应用价值。方法 收集169 例妇科门诊患者阴道分泌物标本同时用阴道炎五联检试剂盒与显微镜镜检，用Kappa检验比较两种方法的一致性。结果 五联检试剂盒与镜检结果比较：过氧化氢与细菌性阴道病（BV）与镜检一致性良好，κ分别为0.61、0.74，总符合率分别为89.90%、92.90%；白细胞酯酶两方法一致性中等，κ=0.58，总符合率为78.70%；真菌两方法一致性差，κ=0.21，总符合率为68.00%，假阳性率为30.50%，假阴性率为39.30%；169例样本中镜检出滴虫1例，而五联检试剂盒未检出。结论 阴道炎五联检试剂盒检测真菌、滴虫结果假阴性、假阳性率较高，需显微镜镜检确认；白细胞酯酶与镜检一直性中等，需结合镜检；过氧化氢和BV与镜检一致性较好，可以满足基层医院临床使用要求。 　　关键词：阴道分泌物；过氧化氢；白细胞酯酶；真菌；细菌性阴道病 　　中图分类号：R711.31；R446.1 文献标识码：A 文章编号：1006-1959（2017）18-0067-03 　　Abstract：Objective To evaluate the clinical application value of the master vaginitis five inspection kit.Methods 169 cases of gynecological patients with vaginitis vaginal secretion and five inspection kit with microscopy，consistent with Kappa test to compare the two methods.Results Five inspection kit and microscopic examination results：Hydrogen peroxide and bacterial vaginosis（BV）and microscopic consistency is good，κ were 0.61，0.74，the total coincidence rate were 89.90%，92.90%；leukocyte esterase two method medium consistency，κ=0.58，the total coincidence rate was 78.70%；Fungi two methods of poor consistency，κ=0.21，the total coincidence rate was 68.00%，the false positive rate was 30.50%，the false negative rate was 39.30%；A total of 1 case of trichomoniasis was detected in 169 samples，while the five test kit was not detected.Conclusion The five inspection kit for detection of fungal vaginitis，trichomonas and false negative results，high false positive rate，need microscope to confirm；Leukocyte esterase and microscopic examination are always moderate，and need to be combined with microscopic examination.The consistency of hydrogen peroxide and BV is better than that of microscopic examination，which can meet the requirements of clinical application in primary hospitals. 　　Key words：Vaginal secretions；Hydrogen peroxide；Leukocyte esterase；Fungi；Bacterial vaginosis 　　阴道分泌物常规检测（俗称“白带常规”）是妇科门诊检查中应用最普遍的一项，是评判女性是否患有阴道炎症的重要诊断依据。传统的检测手段为湿片显微镜检法，主要检测项目包括上皮细胞、白细胞、乳酸杆菌数量、查找真菌、滴虫及线索细胞。该方法简单直接、特异性高，也一
