全身麻醉联合硬膜外阻滞对改善胃癌根治术后患者疼痛的应用价值分析 　　[摘要] 目的 探究不同麻醉方式对改善胃癌根治术后患者疼痛效果。方法 方便选取该院2013年2月―2015年2月期间收治的42例胃癌根治术者作为观察组行全麻+硬膜外阻滞，选同期胃癌根治术42例作对照组行全麻，对比两组术后疼痛程度等。结果 观察组术后VAS评分均低于对照组，P0.05；观察组不良反应率21.4%，低于对照组47.6%，P0.05。结论 该麻醉方式可缓解术后疼痛，减少术后不良反应。 　　[关键词] 全身麻醉；硬膜外阻滞；胃癌根治术；术后疼痛 　　[中图分类号] R4 [文献标识码] A [文章编号] 1674-0742（2016）03（c）-0080-02 　　Analysis of Application Value of General Anesthesia Combined with Epidural Block in Improving the Pains of Patients after Vesssed Gastric Cancer 　　PENG Gui-fang 　　Department of Pain Treatment， Chongzuo Peoples Hospital， Chongzuo， Guangxi Province， 532200 China 　　[Abstract] Objective To discuss the effect of different anesthesia methods in improving the pains of patients after vesssed gastric cancer. Methods 84 cases of patients with vesssed gastric cancer？treated in our hospital from February 2013 to February 2015 were divided into two groups with 42 cases in each， the observation group were treated with general anesthesia combined with epidural block， the control group were treated with general anesthesia， the postoperative pain degrees of the two groups were compared. Results The postoperative VAS score in the observation group was lower than that in the control group， P0.05， the adverse reaction rate in the observation group was lower than that in the control group （21.4% vs 47.6%）， P0.05. Conclusion The anesthesia method can relieve the postoperative pains and reduce the postoperative adverse reactions. 　　[Key words] General anesthesia； Epidural block； Vesssed gastric cancer； Postoperative pains 　　胃癌是临床常见恶性肿瘤之一，目前临床常采用胃癌根治术治疗，然而因该术式是一种开放性手术，涉及区域的神经支配较复杂，由此术后疼痛是胃癌根治术后常见多发症之一，常易发生心血管不良事件、静脉血栓等并发症，严重影响患者的预后情况，故此选择理想的麻醉方式，对改善术后疼痛尤为重要[1]。为更好地保证胃癌根治术患者手术治疗，缓解术后患者疼痛程度，该研究将对2013年2月―2015年2月期间该院收治的42例胃癌根治术患者行全麻联合硬膜外阻滞麻醉，并与同期开展同术式患者42例行单纯全身麻醉，重点比较两组患者术后疼痛程度及用药安全性，旨在探寻胃癌根治书中更为合理的麻醉方式，现报道如下。 　　1 资料与方法 　　1.1 一般资料 　　方便选择2013年2月―2015年2月期间该院收治的42例胃癌患者作观察组，男22例，女20例；年龄35～78岁，平均（48.6±2.3）岁。另选同期同症患者42例作对照组，男23例，女19例；年龄36～79岁，平均（49.5±2.4）岁。均经病理学检查及胃镜检查
