D —二聚体和高敏C反应蛋白对B型主动脉夹层的诊断价值 ［ ］目的探讨D-二聚体和高敏C反应蛋白 对B型主动脉夹层的诊断价值。方法选取2013年6 月至2015年6月来我院就诊的B型主动脉夹层患者 共42例，根据入院检查结果分为B型典型主动脉夹 层和非典型主动脉夹层(壁内血肿和穿透性溃疡)，分 别于发病12、24、72 h, 2周检测患者的D-二聚体及 高敏C反应蛋白水平，分析不同类型B型主动脉夹层 与上述两个指标的关系。结果主动脉夹层发生后患者 D-二聚体及高敏C反应蛋白均不同程度升高。典型组 及非典型组D-二聚体在发病12、24、72 h, 2周血清 水平分别为(875.0± 190.0)、( 1021.0士386.0)、(2056.0 ±912.0)、(6780.0± 1163.0) ng/ml 和(926.0± 108.0)、 (989.0±320.0)、(1478.0±825.0)、(3568.0± 1558.0) ng/mlo典型组及非典型组hs-CRP在发病12、24、72h, 2 周血清水平分别为(25.8±11.4)、(60.9±9.7)、 (110.0±27.5)、(86.4±21.8) mg/L 和(29.1 ±14.5)、(44.7±11.2)、(103.2±25.4)、(79.5±36.3) mg/L。 典型B型主动脉夹层患者D-二聚体水平明显高于非典型患者(P0.05)。结论D-二聚体及高敏C反应蛋白 均有助于主动脉夹层的诊断，D-二聚体有助于鉴别诊 断B型主动脉夹层的类型。 ［关键词］主动脉夹层；D-二聚体；高敏C反应蛋 白 ［ ］R541.9 ［ ］A ［ ］ 1674-4721 (2016) 10 (b) -0035-04 ［Abstract］Objective To investigate the diagnostic value of D-dimer and high-sensitivity C-reactive protein for type B aortic dissection.Methods 42 patients with type B aortic dissection treated by our hospital from June 2013 to June 2015 were selected and they were divided into typical and atypical type B aortic dissection (intramural hematoma and penetrating atherosclerotic ulcer) according to the admission examination results.D-dimer and C-reactive protein level in patients were detected respectively 12 h, 24 h，72 h and two weeks after disease onset.Relationship between different type B aortic dissection and the above-mentioned two indicators was analyzed.Results After incidence of aortic dissection, D-dimer and C-reactive protein level in patients increased to varying degree.D-dimer of typical and atypical type B aortic dissection was (875.0土 190.0) ng/ml, (1021.0±386.0) ng/ml, (2056.0土 912.0) ng/ml, (6780.0± 1163.0) ng/ml and (926.0 ±108.0) ng/ml, (989.0±320.0) ng/ml, (1478.0土 825.0) ng/ml, (3568.0± 1558.0) ng/ml respectively 12 h, 24 h, 72 h and two weeks after disease onset.C-reactive protein typical and atypical type B aortic dissection was (25.8+ 11.4) mg/L，(60.9 + 9.7) mg/L, (110.0±27.5) mg/L, (86.4±21.8) mg/Land (29.1 ±14.5)
