横切口顶入法腹膜外剖宫产临床剖析 　　【摘要】 目的：观察横切口顶入法腹膜外剖宫产的安全性。方法：2011年1月-2014年12月本院妇产科孕产妇3810例，采用剖宫产娩出胎儿1200例。将1200例按照随机数字表法分为治疗组和对照组，每组各600例。对照组采用传统的腹膜内剖宫产手术，治疗组采用横切口顶入法腹膜外剖宫产。观察比较两组孕产妇术中、术后各观察指标及并发症情况。结果：治疗组术中切皮至胎儿出生总时间、总手术时间、术后输液时间、持续留置尿管时间、开始进食时间、肠管排气时间及住院时间均明显短于对照组，术中出血量明显少于对照组，术后发热率明显低于对照组，差异均有统计学意义（P0.05）。结论：横切口顶入法腹膜外剖宫产的术后恢复快，并发症、合并症少，临床效果满意，值得临床推广应用。 　　【关键词】 横切口； 顶入法； 腹膜外剖宫产； 安全性 　　【Abstract】 Objective：To observe the safety of transverse incision jacking method of extraperitoneal cesarean section.Method：1210 cases with caesarean section from 3810 pregnant women in our hospital from January 2011 to December 2014 were selected，1200 cases were randomly divided into the treatment group and the control group，600 cases in each group.The control group was treated with traditional intraperitoneal cesarean operation，the treatment group was treated with transverse incision jacking method of extraperitoneal cesarean section.The intraoperative and postoperative observation index and complications between the two groups were observed and compared.Result：The total time of cut skin to birth，total operation time，postoperative transfusion time，duration of indwelling urinary catheter，and start eating time intestinal exhaust time and hospitalization time in the treatment group were significantly shorter than the control group，intraoperative blood loss was significantly less than the control group，the postoperative fever rate was significantly lower than the control group，the differences were statistically significant（P0.05）.Conclusion：Transverse incision jacking method of extraperitoneal cesarean has faster postoperative recovery，less complications and satisfactory clinical effect，is worthy of clinical popularization and application. 　　【Key words】 Transverse incision； Jacking method； Extraperitoneal caesarean section； Safety 　　First-author’s address：Shandong Caoxian Hospital of Traditional Chinese Medical，Caoxian 274400，China 　　doi：10.3969/j.issn.1674-4985.2015.21.025 　　剖宫产可以有效地挽救孕妇和
