莘庄社区60岁以上高血压患者代谢综合征患病剖析 　　摘 要 目的：分析社区老年高血压人群中代谢综合征（MS）及其各组分的患病现况。方法：对2010年莘庄社区60岁以上老人体检的3 161例高血压患者进行分析，根据国际糖尿病联盟2004年度推荐的有关中国人MS的标准诊断和组分分类。结果：MS患病率为33.5%，男性为29.2%，女性为36.8%。以中心性肥胖+高甘油三酯为主要表现，占总患病人数的45.0%；其次为中心性肥胖+高血糖，占36.6%。结论：社区老年高血压人群中MS的患病率较高，要加强干预。 　　关键词 高血压 代谢综合征 中心性肥胖 老年人 　　中图分类号：R544.1 文献标识码：A 文章编号：1006-1533（2014）04-0043-02 　　Analysis of metabolic syndrome in patients with 　　hypertension aged sixty and over in Xinzhuang Community 　　ZHANG Hongmei， SUN Keyu， ZHANG Xuanzhao， LI Ming， GU Hao 　　（1. Methods： Three thousand one hundred and sixty-one hypertension cases aged 60 and over with hypertension in Xinzhuang Community were analyzed according to the criteria for diagnosis and classification of components recommended by the International Diabetes Federation in 2004 on MS of China’s human. Results： The prevalence of MS was 33.5%， and the male was 29.20% and female 36.80%. MS with central obesity and high blood triglycerides as the common manifestation accounted for 45.0% followed by central obesity and high blood sugar （36.6%）. Conclusion： The prevalence of MS in community elderly hypertensive population is high and intervention should be strengthened. 　　KEY WORDS hypertension； metabolic syndrome； central obesity； old people 　　1 对象与方法 　　1.1 研究对象 　　2010年上海市闵行区莘庄社区60岁以上老人体检中高血压患者3 161例，其中男1 386人，女1 775人，平均年龄69岁（60～93岁）。 　　1.2 诊断标准 　　采用全自动血液生化分析仪检测空腹血糖、肝肾功能、甘油三酯（TG）及低密度脂蛋白（LDL-C）。测量收缩压、舒张压、身高、体重、腹围、臀围。 　　代谢综合征（metabolic syndrome， MS）的诊断采用2004年中华医学会糖尿病分会关于MS的定义，即符合以下3项或以上者：①超重或肥胖 BMI25；②高血压 收缩压≥140 mmHg和（或）舒张压190 mmHg及（或）已确诊为高血压并治疗者；③高血糖 空腹血糖≥6.1 mmol/L和（或）已确诊为糖尿病并进行治疗者；④血脂紊乱 高甘油三酯血症（TG≥1.7 mmol/L）和（或）低高密度脂蛋白血症（男性空腹HDL-C≤0.9 mmol/L，女性空腹HDL-C≤1.0 mmol/L[1]。 　　2
