老年消化性溃疡并上消化道出血局部用药疗效剖析 　　[摘要] 目的 探讨口服凝血酶、云南白药治疗老年消化性溃疡并上消化道出血的临床效果。 方法 选取本院2011年4月～2013年10月收治的85例老年消化性溃疡并上消化道出血患者作为研究对象，随机分成观察组一（35例）、观察组二（30例）、对照组（20例）。对照组在扩容补液、对症支持治疗基础上，根据病情给予奥美拉唑、6-氨基己酸、巴曲亭、奥曲肽等药物常规治疗；观察组一在对照组的基础上给予凝血酶1000 U加生理盐水20 ml口服，每6小时1次；观察组二在对照组的基础上给予云南白药0.5 g加冷开水20 ml口服，每6小时1次，用药后72～96 h观察三组的临床疗效。 结果 观察组一的总有效率为94.28%，观察组二的总有效率为93.33%，对照组的总有效率为85.00%，观察组一与观察组二的总有效率比较，差异无统计学意义（P0.05）。观察组一与观察组二的总有效率显著高于对照组，差异有统计学意义（P0.05）。 结论 凝血酶、云南白药治疗老年消化性溃疡并上消化道出血效果显著，值得临床推广应用。 　　[关键词] 消化性溃疡出血；老年患者；凝血酶；云南白药；局部止血 　　[中图分类号] R573.2 [文献标识码] A [文章编号] 1674-4721（2016）04（b）-0143-03 　　[Abstract] Objective To explore the treatment effect of thrombin，Yunan Baiyao in the treatment of peptic ulcer and upper gastrointestinal hemorrhage in senile patients. Methods 85 senile patients with peptic elcer and upper gastrointestinal hemorrhage from April 2011 to October 2013 in our hospital were selected and randomly divided into the first observation group （35 cases），the second observation group （30 cases） and the control group （20 cases）.According to patient′s condition，the control group was treated with conventional treatment such as omeprazole，6-aminocaproic acid，hemocoagulase and octreotide etc. on the basis of the expansion of fluid infusion，symptomatic and supportive treatment.The first observation group was treated with thrombin 1000 units and 20 milliliter normal saline on the basis of the control group，one time every 6 hours.The second observation group was treated with Yunnan Baiyao 0.5 gram and 20 milliliter cold boiled water on the basis of the control group，one time every 6 hours.The clinical effect of the three groups was compared after treatment 72 to 96 hours. Results The total effective rate of the first observation group was 94.28%，the total effective rate of the second observation group was 93.33%，the total effective rate of the control group was 85.00%.There was no significant difference between the first observation group and the second observation group （P0.05）.Th
