踝关节骨折脱位60例临床剖析 　　[摘要] 目的 探讨骨折脱位治疗的有效方法，总结治疗经验。 方法 将60例踝关节骨折脱位患者随机分为两组，30例作为保守组采取手法复位石膏外固定治疗，30例作为手术组采取手术切开复位内固定，采用Mazur标准评定踝关节功能恢复及通过X线片观察骨折复位情况，同时对两组住院天数与住院费用进行比较。结果 随访患者5个月～1年，两组患者踝关节功能恢复及X线片检查结果差异无统计学意义。保守组住院天数为（7.98±2.12） d，治疗费用为（2 498.6±412.3）元，手术组分别为（16.52±4.58） d、（1 1459.5±412.3）元，两组差异有统计学意义（P ＜ 0.05）。结论 采用手法复位石膏外固定治疗踝关节骨折脱位可达到同手术治疗一样的效果，且保守治疗明显缩短了住院时间，降低了医疗费用，安全性高，是治疗踝关节骨折脱位行之有效的方法。 　　[关键词] 踝关节骨折脱位；固定；保守治疗；骨折 　　[中图分类号] R683.42 [文献标识码] A [文章编号] 1674-4721（2012）06（a）-0053-02 　　The clinical analysis of 60 patients of ankle fractures dislocation 　　WANG Guanghui LI Weiwei YOU Qin LU Daowang GUO Kunying 　　Department of Orthopaedics, Shanwei Yihui Foundation Hospital in Guangdong Province, Shanwei 516600, China 　　[Abstract] Objective To study the fracture dislocation of the effective method of treatment, summarize the treatment experience. Methods Sixty patients with ankle fractures dislocation were choosed and divided into two groups randomly, 30 cases were treated with technique to reset the plaster fixed as a conservative group, 30 cases were treated with open reduction and internal fixation operation as operation group, used the Mazur standard to assess the functional recovery ankle and observed the fracture restoration through the X-ray, and to compare the days in hospital and the charge of be in hospital of the two groups. Results The patients were followed-up for six months-one year, the ankle function recoveration and X-ray examination results of the two groups of patients had no significant difference. Conservative group’s days in hospital were (7.98±2.12 )days, the treatment cost were(2 498.6±412.3 )yuan, the operation group were (16.52 ±4.58) days, (1 1459.5 ±412.3) yuan, the difference between the two groups was statistically significant (P ＜ 0.05). Conclusion The gimmick reset the ankle fractures treated with a plaster cast dislocation can achieve the same effect with surgery, and conservative treatment significantly shortened the hospitalization time
