多形性室性心动过速的心电学特征及临床意义 　　多形性室性心动过速的心电学特征及临床意义 罗昭林　何作云　钟杭美　宋永康　文　芳 　　提要　 为观察多形性室性心动过速 (PV T ) 的临床意义 , 对 18例 PV T 与 34例单形性室性心动过速 (M V T ) 的 心电学和临床特征对比分析后发现 :11PV T 组的室性心动过速 (V T ) ≥ 6阵 日的发生率高 、 频率快 、 Q R S 波的时限 宽 , 两组差异显著 (P 0105) ; 21两组室性早搏总数 、 3级以上和 Q R S 波类型≥ 3种的发生率差异显著 (P 0105) ; 31PV T 组左室射血分数≤ 40%、 左心室肥大 、 心力衰竭和心性死亡的发生率均高于 M V T 组 (P 0105) 。 提示 :有心 脏结构改变和功能障碍的器质性心脏病人患 PV T , 若 V T 阵数多 、 频率快 , 同时又有复杂室性早搏 , 其心性死亡的 危险性大 。 　　关键词　心动过速 , 室性 ; 预后 ; 心电描记 　　Character istics of electrocard iology and cl i n ica l sign if icance 　　of poly m orph ic ven tr icular tachycard i a 　　L uo Z hao -L in , H e Z uo -Y un , Z hong H ang -M ei , et a l . 　　(D ep a rt m en t of Ca rd iology , T he S econd A f f ilia ted H osp ita l of T h ird M ilita ry M ed ica l Col 2 leg e , PL A , Chong qing , 400037) 　　ABSTRCT 　 In o rder to ob serve clin ical singificance of po lym o rp h ic ven tricu lar tachycardia (PV T ) , characteristics of electrocardi o logy and clin ic w ere studied in 18p atien ts w ith PV T and 34p atien ts w ith m onom o rp h ic ven tricu lar tachycardia (M V T ) .
