小剂量芬太尼联合布比卡因用于腰硬联合麻醉在剖宫产术中预防寒战和牵拉痛的临床观察 陈秋宇 唐祖恩(达州市屮心医院麻醉科 四川达州635000) 【摘要】目的 观察应用小剂量芬太尼联合布比卡因用于腰硬联合麻醉在剖宫产 术屮预防寒战与牵拉痛的临床效果。方法 将我院2011年1月到2012年2月间 收录的100例在腰硬联合麻醉下行剖宫产术的患者随机分为观察组和对照组。观 察组蛛网膜下腔阻滞麻醉用药为0.75%布比卡因1.2 ml+芬太尼20mu;g(0.4ml)+ 脑脊液稀释至2.5 ml,对照组蛛网膜下腔阻滞麻醉用药为0.75%的布比卡因 1.2mL+脑脊液稀释至2.5ml。分别观察两组患者寒战与牵拉痛的发生情况，以及 平均动脉压和心率的变化情况。结果 观察组和对照组患者在术前、注药后以及 术毕时间点的平均动脉压和心率比较无明显的差异(P0.05),统计学无意义。 观察组寒战发生率、牵拉痛的疼痛率均明显低于对照组的情况(P0.05),统计 学有意义。结论临床屮应用小剂量芬太尼联合布比卡因用于腰硬联合麻醉能够 预防剖宫产术屮寒战的出现，并有效降低牵拉痛的发生。 【关键词】剖宫产术 腰硬联合麻醉 布比卡因 芬太尼 【中图分类号】R614 【文献标识码】A 【文章编号】2095-1752 (2013) 23-0089-02 A small dose of fentanyl combined with bupivacaine for combined spinal-epidural anesthesia in cesarean section in the prevention of shivering and clinical observation of drag pain 【Abstract] Objective: To observe the application of small dose offentanyl combined with bupivacai ne for combi ned spin al-epidural an esthesia clin ical effect in prevention of shivering during cesarean section and traction pain. Methods: In our hospital from 2011 Janu ary to 2012 February in eluded 100 cases in combi ned spin al-epidural an esthesia for cesarean sectio n were ran domly divided into observation group and control group. The observation group subarachnoid block anesthesia of 0.75% bupivacaine 1.2 ml+ fentanyl 20 mu; g (0.4ml) + cerebrospinal fluid diluted to 2.5 ml, the control group subarachnoid block anesthesia of 0.75% bupivacaine 1.2mL+ cerebrospinal fluid dilution to 2.5ml. The two groups were observed in patients with chills and pull the occurrenee of pain, as well as changes in mean arterial pressure and heart rate. Results: The observation group and the control group were no significant differences in preoperative, after injection and postoperative time points in the mean arterial pressure and heart rate (P gt; 0.05), no statistical significanee.
