PAGE 3 髋关节一过性骨髓水肿综合症的MRI诊断及鉴别诊断 邬小平，刘春英，张东，宁文德，杨军乐，董季平，银小辉 作者单位：710003 西安市中心医院医学影像中心（邬小平，刘春英，宁文德，杨军乐，董季平，银小辉）；西安市红十字会医院放射科（张东） 作者简介: 邬小平（1975-），男，重庆市垫江县人，汉族，硕士，主治医师，研究方向：MRI影像诊断，发表论文5篇，E-mail szping518@163.com ?通信作者：邬小平 摘要:目的 探讨髋关节一过性骨髓水肿综合症MRI表现 ,提高其诊断及鉴别诊断水平。方法 结合文献对13例髋关节一过性骨髓水肿综合症的MRI表现进行回顾性分析。结果 双侧髋关节同时受累4例，单侧9例（左侧6例，右侧3例）；病变累及股骨头、股骨颈13侧，累及股骨头、股骨颈及股骨粗隆4侧。病变在T1WI呈等或略低信号13例；T2W1呈高信号或略高信号11例，等信号2例；T2WI脂肪抑制病变均呈高信号。合并关节积液8例，呈长 TI、长T2信号。股骨头形态均无异常。结论 正确认识髋关节一过性骨髓水肿综合症的 MRI表现，有利于正确诊断，减少误诊。 关键词:骨髓;水肿;股骨;磁共振成像 MR imaging diagnosis and differential diagnosis of transient Bone marrow edema syndrome of the hip Wu Xiao-ping, Liu Chun-ying, Zhang Dong, Ning Wen-de, Yang Jun-le, Dong Ji-ping, Yin Xiao-hui Medical Imaging Centre, Xi`an City Central Hospital, Xi`an 710003 China Abstract : Objective To study MR imaging features of transient Bone marrow edema syndrome of the hip and to improve the ability of diagnosis and differential diagnosis. Methods MR images of transient Bone marrow edema syndrome of the hip in 13 patients were retrospectively reviewed with literature review. Results The bilateral hips were affected in 4 patients. The single hip was affected in 9 patients, the left hip in 6, the right hip in 3. The MR images demonstrated low signal intensity in all 13 patients on T1WI. And normal signal intensity 2 patients, medium or high signal intensity 11 patients on T2WI. and high signal on T2 fat suppressed or STIR images in all 13 patients. The bone marrow edema pattern involved the femoral head and neck in 13 hips, the femoral head and neck and the intertrochanteric region in 4 hips. A small joint effusion Was observed in 8 hips on T2WI. The configuration of femoral heads were normal. Conclusion The correct recognization of the MR imaging manifestations of transient Bone marrow edema syndrome of the hip can improve the ability of diagnosis and differential diagnosis .
