May be instances (although rare) where you feel resistance but filter isn’t fully sheathed – may be due to large embolic load Okay to have filter outside of sheath, but make sure filter loop is proximal to marker band on sheath New Slide If any resistance is met during retraction of the sheathed FilterWire EZ System, slightly advance the sheathed FilterWire EZ System and rotate the sheath before continuing to retract. Slowly and carefully retract the entire system until the tip of the retrieval sheath is adjacent to the tip of the guiding catheter or guiding sheath. If there is any resistance, retract the guiding catheter and FilterWire EZ System together. This is done in case there is a large amount of embolic material captured in the filter that may not fit inside the guiding catheter. Remove the FilterWire EZ System from the patient. If a rotating hemostasis valve is used, ensure the valve is wide open during retraction. Achieve Good Guide Catheter Support: Loss of guide catheter engagement may cause accidental filter movement. Ensure Sufficient Landing Zone: Allowing extra room for small inadvertent filter movement is good practice to prevent the filter from coming back into the stent or lesion. Also, be careful not to jam the tip of the balloon or SDS catheter onto the filter’s catheter stop, as this may cause wedging and wire lockup, leading to unintentional filter retraction. Minimize Wire Movement: Secure the filter loop position throughout the procedure to prevent inadvertent movement. In particular, visualize the filter loop under fluoroscopy during catheter exchanges to ensure the filter is not unintentionally retracted into the stent. Achieve Full Stent Apposition: Freshly deployed stents that are not well apposed can be difficult to cross with retrieval devices. Post-dilatation will help ensure good wall apposition and will help facilitate filter removal. If post-dilatation is not successful in permitting recrossing of the stent, try th
