Methods 34 patients diagnosed as but fracture of thoracolumbar vertebra with spinal stenosis were cured by ilium autografting with vertebral canal posterior wall plastic operation combining pedicle screw fixation since 1996. Result For 18 months -5 yea following up , satisfactory recovery of vertebral bodys altitude, ideal correction of cobbs angle were observed, at least one grade recover was observed in all cases except 2 patients with pre-operative A degree of frankel classification. No compression in dura mater and nerve roots was detected ,also no pain symptoms in waist and lower limb; broader post-operative sagittal diamete of vertebral canals were obtained than nearby vertebral canals, average ratio of sagittal diamete between operative segments and nearby unoperation segments were 138%～160%. Conclusion It is an ideal operation for applying ilium autografting with vertebral canal posterior wall plastic operation combining pedicle screw fixation to treat but fracture of thoracolumbar vertebra with spinal stenosis, it is a simple and safe operative technique to entirely decompress in duramater and nerve roots and enlarge bore of vertebral canal there
