临床医学论文-胃癌组织中基质金属蛋白酶-9和整合素β3 mRNA表达的临床意义 ???????????? 作者：杨月明 叶再元 赵仲生 王元宇? 【摘要】? 目的 研究基质金属蛋白酶-9（MMP-9）和整合素β3（Integrin β3）mRNA在胃癌组织中的表达及与临床病理参数、肿瘤微血管密度、生存期的关系。 方法 应用原位杂交和免疫组化，检测105例胃癌组织中MMP-9、Integrinβ3 mRNA和CD34的表达。结果 MMP-9和Integrinβ3 mRNA 阳性表达率分别为58.10％和60.95％，其阳性表达与胃癌生长方式、浸润深度、脉管侵犯、淋巴结及远处转移均密切相关（P均＜0.01）； MMP-9、Integrin β3 mRNA 的阳性表达病例的平均血管数均高于阴性表达病例的平均血管数（P均＜0.05）；MVD值分别与MMP-9和Integrin β3 mRNA的表达水平呈正相关（P均＜0.05）； MMP-9和Integrinβ3 mRNA的阳性表达以及MVD值≥39.5病例的5年生存率显著低于阴性表达和MVD 值＜39.5的病例；MMP-9和Integrin β3 mRNA表达存在显著相关性（P＜0.05）。 结论 MMP-9、Integrinβ3可促进胃癌血管生成，并参与肿瘤浸润转移过程，可以作为反映胃癌生物学行为和判断预后的有效指标。 【关键词】? 胃癌 基质金属蛋白酶-9 整合素β3 转移 预后 　　【Abstract】Objective? To investigate the expression of matrix metalloproteinase-9 (MMP-9) and integrin beta-3 mRNA in gastric carcinoma and to find the correlation between the expression rate of them and clinicopathologic index，microvessel density and survival time of gastric carcinoma patients.? Methods? In situate hybridization and immunohistochemical techniques were used to detect the gene expression of MMP-9, Integrin beta-3 and CD34 in 105 gastric carcinoma specimens.? Results? In situ hybridization revealed that positive expression rate of MMP-9 and Integrin beta-3 mRNA was 58.10％ and 60.95％，respectively. There was a significant correlation between the expression of them and growth pattern , depth of tumor invasion，vessel invasion , lymph node and distant metastasis (P＜0.01). Mean tumor MVD of patients with positive mRNA expression was significantly higher than those with negative mRNA expression(P＜0.05). In addition, MVD value was positively correlated with the expression level of MMP-9 and Integrin beta-3 mRNA (P＜0.05). The five year survival rate of patients with positive mRNA expression and mean MVD value≥39.5/mm2 were significantly lower than those of patients with negative mRNA expression and mean MVD value＜39.5/mm2; The expression of MMP-9 was significantly corelated with the expression of integrin β3 (P＜0.05).?
