ChineseMedicatJournal2009,122(1912325—2329 2325 Originalarticle Novelparameterforassessmentofleftatrialsizeinpatientswith hypertension：ratioofleftatrialvolumetoleftventricularvolume LIYue，CHAILiang，ZHANG YunandLILei Keywords：hypertension；leftatrium；leftventricular；cardiacvolume；echocardiography Background LeftatriaIenlargementhasbeen suggested asa more robustmarkerofdiastolic dysfunction．W e hypothesizethattheratioofIeftatriaIvolumetoIeftventricularvolume(LAV，IVV)maybemorereasonabletoreflectIeft atriaIenlargementinthepatientswith hypertension．becausehypertensivepatientshaveacharacteristicofconcentric remodelingofthe leftventriclewhich isoftenaccompaniedwith diastolicdysfunction．Theaim ofthis studywasto determine iftheLAV／LV-V canbeusedasanew parameterto assess Ieftatrialsize Inhypertensivepatientsandthe relationshipbetweentheL | Vanddiastolicdysfunction Methods Ninety-one patients with hypertension and forty·three normaIcontrolswere studied．The hypertensive patientswereassignedtothenormalwall(NW)andhypertrophicwall(Hw)groups．Theleftatrialdiameter(LAD)，LA、 Ieftatrialvolumeindex(LAY．)LVVandLAV／LVVweremeasuredandcalculatedby2一dimensionaIechocardiographyand reaItime3．dimensionaIechOcardiOgraDhV．AIIoftheaboveparameterswereusedtoevaluatethesizeoftheleftatrium． TheratioofpeakEvelociytofmitraIvalveinflowtopeakE’velociytofIateralmitralannulus(E／E)wasmeasuredbypulse DopplerandtissueDoppler．Thisparameterwasusedtoevaluatediastolicfunction． Results TheLAD，LAY,LA、，l，LAV／LVV andE／E inhypertensivegroupsweresignificantlyhigherthanthose inthe normalgroup(P0．05or0．01)，andthoseintheHwgroupweresignificantlyhigherthanthoseintheNwgroup(P0．05 0r0．011．TheE／E hadapositivecorrelationwithL LAViandLAV／LVV ThecorrelationcoefficientbetweenE／E and | VwasrelativelyhigherthanthatbetweenE／EtandLADorL ＼ Conclusion LAV／LVV maybeused
