When pulse wave reaches the brachial artery, pulse wave transmission distance on the direction of heart-ankle is far greater than that on the heart-brachial direction. In the formula for brachial-ankle pulse wave velocity (baPWV), the distance longer than the actual distance is divided by a relatively shorter duration, so the result PWV would be faster than the actual value. Statistical analysis shows that baPWV overstates not only actual pulse wave speed but also reverses the real pulse wave velocity. More studies on baPWV will only bring us trouble. Therefore, we suggest that heart-ankle pulse wave velocity should draw our attention.
