反流性食管炎胃肠检查与胃镜对照研究_临床医学论文 反流性食管炎胃肠检查与胃镜对照研究_临床医学论文 作者：万向荣 张南征 丁心敏 江兴松 【关键词】 胃肠检查 　　【摘要】 目的 研究数字胃肠造影与胃镜检查在反流性食管炎诊断中的价值。 方法 采用数字胃肠造影与胃镜分别对406例反流性食管炎进行检查，并对二种检查结果进行对比分析。 结果 406例反流性食管炎中，数字胃肠造影的表现特征是:早期103例表现为功能性改变，钡剂滞留，食管排空缓慢，出现异常收缩波。中期245例表现为黏膜增粗，呈条状，颗粒状，部分病例有小溃疡形成。晚期58例表现为食管黏膜明显增粗，紊乱，有多个小溃疡，管腔狭窄，钡剂通过受阻，Bar-ret食管形成。胃镜检查食管黏膜充血406例，糜烂融合成片310例，溃疡96例，食管腔狭窄58例。 结论 数字胃肠检查对食管功能性变化，形态与病变范围，胃对食管的影响较清楚，观察也较全面。胃镜对食管炎腔内病变显示较为理想，特别是对早期食管炎病变的观察，同是亦可做病理检查定性。 　　关键词 反流性食管炎 胃肠检查 胃镜诊断 　　Comparison between reflux esophagitis gastrointestinal series and gastroscopy:a report of406cases 　　【Abstract】 Objective To study the value of diagnosis to reflux esophagitis by gastrointestinal series and gas-troscopy.Methods 406cases of esophagitis were checked by gastrointestinal series and gastroscopy，analysing the re-sults of two methods.Results Among406cases of esophagitis，the typical characteristic of cases by gastrointestinal series were:103cases with early illness history showed the functional change of esophagus，the dose of barium kept in the stomach，the empty of the esophagus was slow and the special wave of contract occurred;245cases with middle ill-ness history showed mucous membrane widened，pellet，some cases formed light ulcer，58cases with long illness history showed the wideness of mucous membrane increased obviously in esophagus，disordered，there were a lot of small ul-cers，the esophagus turned to be narrow and the dose of barium went through with difficulties.Barret formed.Among406cases of bloody mucous membrane in esophagus by gastroscopy，310cases were erosion combined into pieces，96cases of ulcer，58cases as narrowness of esophagus.Conclusion The digital gastrointestinal series is good to diagnose the functional change，shape and scope of esophagus.It is clear to observe the effect of stomach to esophagus.Gas-troscopy is an ideal method to observe the pathological changes，especially，it can observe the early pathological changes on esophagus，it also helps to diagnose on the pathology.
