中国实用医药2015年1月第10卷第2期 ChinaPracMed，Jan2015，Vo1．10，No．2 · 21 · Madelung病临床特征分析及治疗方法的探讨 何 晶晶 关丽梅 苏文玲 【摘要】 目的 分析Madelung病的临床特征并探讨其治疗方法。方法 结合文献分析 1例Madelung 病患者的临床表现、CT影像检查、诊断、治疗及预后情况。结果 患者主要表现为身体上半部脂肪弥 漫性沉积 ，颈部畸形和活动受限，有饮酒嗜好 ，并伴有多种内科疾病 ，如酒精性肝病、高血压病、高尿酸 血症、甲状腺功能减退等。经手术治疗和内科治疗后，现恢复良好，无复发及恶变倾向。结论 Madelung 病具有特征性CT表现 ，即颈项部、下枕部、上背部皮下有大量脂肪组织蓄积增厚，其密度和信号强度 与正常脂肪相同。本病可采用综合治疗的方法 ，除戒酒外，可将吸脂术与外科手术相结合，同时积极治 疗内科疾病 ，这样有利于美观、减少复发及并发症，且改善预后。 【关键词 】Madelung病 ；脂肪过多症 ；多发对称性 DOI：10．14163／j．cnki．11-5547／r．2015．02．010 Clinicalcharacteristicsanalysisandtreatmentmethod investigationofMadelungSdisease HEJing— ring,GUANLi—met，SUWen—ling．DepartmentofOtolaryngologyHeadandNeckSurgery,TheFirstAffiliated HospitalofXiamenUniversity,Xiamen361000,China． 【Abstract】Objective Toanalyzetheclinicalcharacteristicsandexplorethetreatmentmethodof Madelung’Sdisease．M ethods Theanalysiswasmadeontheclinicalmanifestations，CTimagingexamination， diagnosis，treatment，andprognosisof1patientwithMadelung’Sdisease，alongwithliteraturereview． Results Themanifestationsofthepatientwerediffusefattymassintheupperbody，neckdeformity，restricted motion，drinkinghabit，andseveralinternalmedicinediseases，suchasalcoholicliverdisease，hypertension， hyperurieemia，andhypothyroidism．Aftersurgeryandmedicaltreatment，thepatientrecoveredwellwithout recurrenceandmalignanttransformation．Conclusion TheCTimagingofMadelungdiseaseshowsthefeature ofaccumulatedfatdepositedatsubcutaneoussitesoftheanteriororposterioroftheneck，theloweroccipitalia andtheupperback．Thedensityandsignalintensityoftheexcessfataresimilarasnomr alaft．Comprehensive treatmentscanbeappliedforthetreatmentincludingtemperance，combinationliposuctionandlipectomy，and activemedicaltherapy．Thatcanbenefitpatient’Soutlook，decreasetherecurrenceandcomplications，andget favourableprognosis． 【Keywords】Madelung’Sdisease；Lipomatosis；Multiplesymmetry Madelung病 (madelung’sdisease)也称多发对称性脂肪过 大脂肪组织 ，大小约20am×
