E Cadherin在进展期胃癌转移淋巴结中的再表达及意义 　　作者：刘津,赵琪,张国建,段国强,薛平,宋子军　　作者单位：050000 石家庄市, 河北医科大学第二医院胃肠外科 　　【摘要】 目的 检测进展期胃癌及其转移淋巴结中上皮型钙粘蛋白(E Cadherin ,E Cad)及其mRNA表达，从而为胃癌的诊断、治疗及预后提供依据及线索。方法 采用免疫组织化学染色的方法观察比较E Cad 在进展期胃癌原发灶及转移淋巴结中的表达; 同时用逆转录 多聚合酶链反应(RT PCR)方法检测上述标本中E Cad 基因的表达，并对其和临床病理参数的关系进行分析。结果 (1)在38例胃癌原发灶中94.7%(36/38) E Cad表达程度减低, 21.1%(8/38)胃癌转移淋巴结中E Cad再表达阳性,其中高分化胃癌的再表达阳性率为35.3%(6/17);中低分化胃癌的再表达阳性率为9.52%(2/21)。高分化癌相对于中低分化癌更易出现E Cad的再表达阳性,两者比较差异有统计学意义(P0.05)。(2)RT PCR结果显示，38例原发灶组织及相应转移淋巴结均检测到不同程度的E Cad mRNA表达。E Cad mRNA在胃癌的表达量(0.737±0.010)明显低于胃正常黏膜表达量(0.799±0.018)，两者比较差异有统计学意义(P0.01);结合临床病理参数分析，高分化胃癌转移淋巴结中E Cad mRNA表达高于低分化胃癌转移淋巴结，管状腺癌转移淋巴结中E Cad mRNA表达高于粘液腺癌转移淋巴结。结论 E Cad基因变化与肿瘤的发生、浸润、转移有关, E Cad基因治疗可作为胃癌治疗的靶点。 　　【关键词】 胃肿瘤 上皮钙粘蛋白 免疫组织化学染色 逆转录多聚合酶链反应 　　Expression of E Cadherin in human advanced gastric carcinoma and its clinical significance LIU Jin, ZHAO Qi,ZHANG Guojian,et al.The Second Hospital of Hebei Medical University, Shijiazhuang 050000 　　【Abstract】 Objective To investigate the expressions of epithelial cadherin (E Cad) protein and mRNA in human advanced gastric carcinoma and metastasis lymph node, and to provide the evidence and clue for diagnosis and treatment of gastric carcinoma. Methods Immunohistochemical staining was used to examine the expression of E Cad in 38 cases of human advanced gastric carcinoma tissues and metastasis lymph node. E Cad mRNA was evaluated in the above mentioned samples by semi quantitative RT PCR, and the correlation between E Cad mRNA expression and its related clinicopathological parameters was analyzed.Results Immunohistochemical staining showed that the abnormal expression rate of E Cad herin was 94.7% (36/38), and the expression rate of E Cad in metastasis lymph node was 21.1% (8/38). The expression rate in well differentiated gastric carcinoma was significantly higher than that in poorly differentiated gastric carcinoma.
