_______________________________________________________________________________ 新生儿先天性甲状腺功能低下症替代治疗前后左心功能的变化 毛姗姗，赵正言，蒋国平 （浙江大学医学院附属儿童医院，浙江杭州，310003 ） [摘要] 目的 评价先天性甲状腺功能低下症（CH ）新生儿的左心收缩和舒张功能改 变，以及 L-T4 替代治疗后左心功能的变化，并探讨其与血甲状腺激素水平的相关性。方法 对 38 例 CH 新生儿于 L-T4 治疗 1 月前后和 30 例对照组进行超声心动图检查，并对血甲状 腺激素水平和心功能指标行相关性分析。结果 CH 组替代治疗前左心收缩功能指标 LVEF 、 LVFS 、s 、MAD 与左心舒张功能指标 E 、E /A 、e 、e /a 较对照组显著下降 （P 均0.01 ）， m m m m、 m m m 且与 TT4 呈正相关（P 均0.01 ），与 TSH 呈负相关（P 均0.01 ）。CH 组用 L-T4 替代治疗 1 月后，上述指标较治疗前显著上升（P 均0.01 ）。结论 先天性甲状腺功能低下症新生儿常 伴有左心功能下降，收缩和舒张功能均累及，早期进行 L-T4 替代治疗可不同程度地改善受 损的左心功能。 [关键词] 先天性甲状腺功能低下症；左心功能；新生儿；替代治疗；L-T4 Left ventricular function in congenital hypothyroidism neonates before and after L-T4 therapy.
