耳穴贴压配合柴胡解郁汤治疗卒中后抑郁患者60例临床观察 　　摘要：目的：观察耳穴贴压配合柴胡解郁汤治疗卒中后抑郁（post-stroke depression， 简称PSD）的临床疗效，并对5-HT递质水平进行监测和评价。方法：将120例PSD患者随机分为治疗组60例，对照组60例，对照组给予口服舍曲林片治疗。治疗组给予耳穴贴压配合柴胡解郁汤治疗，治疗8周后评价疗效，分别在治疗第2、4、8周给予记录HAMD 评分、测定血清5-HT递质水平观察疗效。结果： 治疗后治疗组临床疗效优于对照组（Plt;0.05），差异有统计学意义;HAMD 评分较治疗前明显改善（Plt;0.01），且组间相比（Plt;0.05），差异有统计学意义;对照组治疗2周后HAMD 评分与治疗前相比（Pgt;0.05），差异无统计学意义;4周后与治疗前比较（Plt;0.05），差异有统计学意义;同一时间点组间比较： 治疗第2周末治疗组HAMD评分与同一时间点对照组比较差异有统计学意义（Plt;0.05）。结论： 耳穴贴压配合柴胡解郁汤治疗中风后抑郁疗效明显，其作用机制可能与其升高PSD患者血清5-HT递质水平密切相关。 　　关键词：卒中后抑郁，柴胡解郁汤;耳穴贴压;舍曲林片;HAMD;5-HT 　　Abstract： Objective： To observe the clinical curative effect of auricular points plaster therapy combined with Chaihu Jieyu Decoction in the treatment of post stroke depression （PSD） and to monitor and evaluate the level of 5-HT transmitter. Methods： 120 cases of PSD were randomly divided into treatment group （60 cases） and control group （60 cases）. The treatment group was given auricular points plaster therapy combined with Chaihu Jieyu Decoction while the control group was given oral sertraline treatment. The curative effect was evaluated after 8 weeks’ treatment. The score of HAMD was recorded and the level of 5-HT transmitters in serum was measured after 2， 4， 8 weeks’ treatments， respectively. Results： After treatment， the clinical effect of treatment group was better than that of the control group （Plt;0.05）， the difference was statistically significant; the HAMD score was significantly improved than pretherapy （Plt;0.01）， and the difference between groups （Plt;0.05） was statistically significant. Compared to the control group， the score of HAMD （Pgt;0.05） after 2 weeks showed no significant difference while showed significant difference after 4 weeks （Plt;0.05）. The comparison of the two groups at the same time nodes： the HAMD score of the treatment group had significant difference compared to that of the control group （Plt;0.05） after 2 weeks’ treatment.
