保留盆腔自主神经的全直肠系膜切除术治疗直肠癌效果观察 　　doi：10.3969/j.issn.1007-614x.2014.12.30 　　摘 要 目的：探讨保留盆腔自主神经的全直肠系切除手术治疗直肠癌的临床疗效。方法：收治直肠癌患者80例，随机分为治疗组和对照组，每组40例。治疗组采用保留盆腔自主神经的全直肠系膜切除手术进行治疗，对照组运用传统的根治手术进行治疗，对比分析两组的临床疗效。结果：治疗组的手术时间以及手术时患者的出血量显著的少于对照组（P0.05）。手术后对患者进行随访，出现吻合口瘘、局部复发、远处转移、死亡、排尿功能障碍、勃起功能障碍和射精功能障碍的治疗组的人数分别为2例、4例、6例、4例、5例、6例、3例，对照组出现的人数分别为6例、10例、11例、7例、10例、12例、7例，治疗组的人数均少于对照组（P0.05）。结论：采用保留盆腔自主神经的全直肠系模切除术治疗直肠癌的临床效果显著，复发率低，手术后患者的生活质量显著提高。 　　关键词 盆腔自主神经 全直肠系膜切除术 临床效果 　　Effect observation of total mesorectal excision with pelvic autonomic nerve preservation in the treatment of rectal cancer 　　Lin Zhiyang1，Zhang Weijian2 　　The Fourth Department Surgery，the Peoples Hospital of Chenghai District，Shantou City，Guangdong 515800l 　　Department of Gastrointestinal Surgery，the First Peoples Hospital of Guangzhou City，5100002 　　Abstract Objective：To explore the clinical effect of total mesorectal excision with pelvic autonomic nerve preservation in the treatment of rectal cancer.Methods：80 patients with rectal cancer were selected.They were randomly divided into the treatment group and the control group with 40 cases in each.The treatment group was treated by total mesorectal excision with pelvic autonomic nerve preservation.The control group was treated by traditional radical operation treatment.We compared the clinical efficacy of the two groups.Results：Through the comparative analysis of the clinical efficacy of the two groups，the operation time and bleeding volume during operation of the treatment group were less than that of the control group（P0.05）.The patients were followed up after operation，there appeared anastomotic fistula，local recurrence，distant metastasis，death，voiding dysfunction，erectile dysfunction and ejaculatory dysfunction number.The number of the treatment group were respectively 2，4，6，4，5，6，3 cases，and the number of the control group were respectively 6，10，11，7，10，12，7 cases.The number of the treatment group was significantly less than that of the control grou
