围术期患者预防性应用抗菌药的分析 　　 　　[摘要] 目的 了解围术期病例抗菌药物的应用情况，评价其用药的合理性，以指导临床合理使用抗菌药。 方法 采用回顾性调查方法，对本院233例围术期病历应用抗菌药物的品种、联合用药情况、用药时间、疗效等进行调查，并用世界卫生组织推荐的以限定日剂量（DDD）及药物利用指数（DUI）为指标评价医院用药是否合理。 结果 联合用药占34.76%，二联用药占26.61%，三联用药占8.15%，其中，二联用药以氨基糖苷类联合喹诺酮类为主，统计的12种抗菌药物中有9种DUI≤1.0，有3种DUI稍微超过1.0。 结论 本院围术期抗菌药使用基本合理。 　　[关键词] 围术期；预防性；抗菌药物；分析 　　[中图分类号] R969.3[文献标识码] A[文章编号] 1674-4721（2014）03（c）-0141-04 　　 　　Analysis of prophylactic application of antibacterials in perioperative period 　　CAI Lan 　　Department of Pharmacy,Dalingshan Hospital of Dongguan City in Guangdong Province,Dongguan 523820,China 　　[Abstract] Objective To understand the application of antibacterials in perioperative patients and to evaluate the rationality of drug use to guide clinical rational use of antimicrobials. Methods The variety of antibacterials,combined use of drugs,medication time and curative effect of 233 perioperative medical records in our hospital were surveyed by retrospective survey method,the indicators of defined daily dose (DDD) and the drug utilization index (DUI) recommend by WHO were used to evaluate whether the drug rational. Results Combined use of drugs accounted for 34.76%,two drugs combination accounted for 26.61%, three drugs accounted for 8.15%, among them,two drugs combination was mainly aminoglycosides combined with quinolones,and in 12 kinds of antibacterials in statistics,there were 9 kinds of DUI＜1.0,there were 3 kinds of DUI slightly more than 1.Conclusion The perioperative use of antibiotics in our hospital is reasonable. 　　[Key words] Perioperative period;Preventability;Antibacterials;Analysis 　　抗菌药物的预防性应用在医药各领域中占有重要的地位。抗菌药物占医院用药金额的比重逐年增长，特别是近年来，新的抗菌药物的增加，为治疗感染性疾病开辟了新的篇章，使许多严重感染得到有效控制，但临床上不合理地预防性应用抗菌药物会导致医疗资源的浪费，更有可能带来细菌耐药性增加等严重问题[1]。为避免发生感染，根据《围手术期预防应用抗菌药物指南》[2]，手术时严格遵守无菌操作规程甚为重要，围术期合理地预防性应用抗菌药物，对降低术后感染率，也是十分必要的[3]，但能否安全、有效、经济、适当地应用抗菌药物是一项综合管理工作，有待各个部门的联合把关[4]。为了了解本院围术期抗菌药物的应用情况，本研究对本院233例围术期患者预防性应用抗菌药物的情况进行分析，为合理应用抗菌药物提供参考依据。 　　1 资料与方法 　　1.1 一般资料 　　随机抽取本院2011年12月~2012年
