慢性荨麻疹患者甲状腺自身抗体检测及自体血清皮肤试验的意义 胡小平1 ,谢谦2 ,于波1 ,邵勇1，张杰1，钟绮丽1 ,马刚1 （1 北京大学深圳医院皮肤科；2 北京大学深圳医院内分泌科 广东 深圳 518036） [摘要] 目的:通过检测指标分析慢性荨麻疹和甲状腺功能亢进的关系，探讨甲状腺自身抗体在慢性荨麻疹发病中的作用和自体皮肤血清试验的意义。方法:研究对象包括100例慢性荨麻疹患者，68例甲亢合并慢性荨麻疹患者，60例甲状腺功能亢进患者和60例健康人群，回顾性分析四组患者的临床资料，检测游离甲状腺素（FT3、FT4）、高敏促甲状腺素（sTSH），甲状腺自身抗体（TGAb、TPOAb、TRAb），并进行自体血清皮肤试验（ASST），比较各组指标间的差异性。结果:慢性荨麻疹组TGAb、TPOAb、TRAb和ASST阳性率均显著高于对照组；甲亢合并慢性荨麻疹组TGAb、TPOAb、TRAb阳性率均显著高于慢性荨麻疹组，两组ASST阳性率有显著性差异；甲亢组TGAb、TPOAb阳性率和甲亢合并慢性荨麻疹组差异无显著性，TRAb和ASST阳性率有显著性差异；慢性荨麻疹组sTSH水平和对照组有显著性差异。结论:甲状腺自身抗体在慢性荨麻疹发病中可能起一定的作用，ASST在甲亢合并慢性荨麻疹患者中阳性率更高。[关键词] 慢性荨麻疹；甲亢；甲状腺自身抗体；自体血清皮肤试验 Detection of Thyroid Autoantibodies and Autologous Serum Skin Test in Patients with Chronic Urticaria Hu Xiao-ping,Xie Qian,Yu Bo,Shao Yong,Zhang Jie,Zhong Qi-li,Ma Gang (Department of Dermatology,Department of Endocrinology,Peking University ShenZhen Hospital,Shenzhen 518036,China) [Abstract] Objective:To investigate the level of thyroid autoantibodies and autologous serum skin test in patients with chronic urticaria (CU).Methods:The subjects are divided into four groups including 100 CU patients, 68 CU combining hyperthyroidism patients,60 hyperthyroidism patients and 60 controlling cases.By measuring the items such as free thyroxine three and four(FT3,FT4), thyrotropin(TSH), thyroid autoantibodies and autologous serum skin test(ASST),the differences of four groups will be compared.Results:There are significant differences about positive ratio of thyroid autoantibodies and ASST between CU and controlling group and CU combining hyperthyroidism. Positive ratio of TRAb and ASST in CU combining hyperthyroidism group are higher than those of hyperthyroidism group.The level of sTSH in CU group are lower than controlling group.Conclusions:Thyroid autoantibodies show possible role in pathogenesis of chronic urticaria.
