腹腔镜在治疗剖宫产后子宫瘢痕妊娠中的应用价值 　　[摘要] 目的 探讨腹腔镜在治疗剖宫产后子宫瘢痕妊娠（CSP）中的临床效果、安全性及其应用价值。方法 选取31 例剖宫产瘢痕妊娠患者为研究对象，分为观察组和对照组。观察组患者实行腹腔镜行妊娠病灶切除术及子宫修补术，对照组患者行中转开腹行病灶楔形切除及子宫修补术。结果 观察组显效10例，有效5例，无效1例，总有效率为93.6%；对照组显效6例，有效5例，无效4例，总有效率73.3%，两组疗效比较差异具有统计学意义（P0.05）；在治疗I 型病例时，观察组疗效明显优于对照组，差异具有统计学意义（P0.05）。 结论 腹腔镜技术在子宫切口瘢痕妊娠的治疗中具有比较重要的意义。腹腔镜具有术中出血少、疼痛轻、术后恢复快、住院时间短、可保留患者生育功能、切口美观等优越性，可有效控制创面，提高患者保留生育能力的概率，是异位妊娠首选的手术方式，值得推广。 　　[关键词] 腹腔镜；剖宫产后子宫瘢痕妊娠 　　[中图分类号] R714.2 [文献标识码] B [文章编号] 1673-9701（2013）36-0159-02 　　Application value of laparoscope in the treatment of cesarean scar pregnancy 　　WANG Hongling 　　Shenyang Women’s and Children’s Hospital， Shenyang 110000， China 　　[Abstract] Objective To evaluate the clinical effect， safety and application value of laparoscope in the treatment of cesarean scar pregnancy（CSP）. Methods Thirty-one cases of patients with cesarean scar pregnancy were selected as the research object， they were randomly divided into the observation group and the control group， the patients in the observation group were treated with laparoscopic pregnancy lesion resection and uterus repair， while the control group were treated with laparotomy of lesion wedge resection and uterus repair. Results In the observation group， 10 cases were markedly effective， 5 cases were effective， 1 case was ineffective， and the total effective rate was 93.6%；In the control group， 6 cases were markedly effective， 5 cases were effective， 4 case were ineffective， and the total effective rate was 73.3%， there were significant differences between the two groups（P0.05）； In the treatment of type Ⅰ， the curative effect of observation group was obviously better than that of control group（P0.05）. Conclusion Laparoscopy has important significance in the treatment of cesarean scar pregnancy.
