Theobjectiveofthepresentstudywastoinvestigatewhetherego-dystonicandego-syntonicsuicidalideationoccurredatdi?erent frequenciesduringantidepressanttherapy.Ablindevaluationhasbeenperformedonrecordsof100suicideswithaprimarysevere depressionand100matchedcontrols,admittedtotheDepartmentofPsychiatry,Lund,Sweden.Ego-dystonicsuicidalideation wasmorecommonlyreportedduringadequatetreatmentascomparedtoego-syntonicideation(P=.004).Menwhocommitted suicideduringadequateantidepressanttherapymoreoftenreportedego-dystonicsuicidalideationearlierintheirlivescompared withthosewhowerenottreated(P =.0377).Thismayindicatethattreatmentfailureforego-dystonicideationwasaprecursor oftheirsuicides.Consequently,ego-dystonicideationseemstoshowapoorerresponsetoantidepressanttherapyascompared toego-syntonicideation,whichmaybemoredirectlyrelatedtodepression.Ego-dystonicideationisproposedtoberelatedto depressivepsychosis. 1.Introduction Thepresentstudyusesaphenomenologicaldistinction between ego-syntonic and ego-dystonic suicidal ideation basedonpreviousstudiesinourgroup.Wehavefoundthat suicidalideation“beyondone’sownwill”(i.e.,ego-dystonic) wasrelatedtocompleteds
