Silentcerebralwhitematterlesions(WMLs)areacommon?ndingonmagneticresonanceimagingofthebrainintheelderly. However,inpatientswithhypertension,WMLstendtooccurearlierinlifeandappeartobemoresevere.Thereisabodyof evidencethatsupportstheideathatWMLsinasymptomatichypertensivepatientsshouldbeconsideredasilentearlymarkerof braindamage.Itisknownthatambulatorybloodpressuremonitoring(ABPM)correlatesmorecloselywithhypertension-related organdamagethano?cebloodpressure.Thispaperfocusesontheassociationsbetweenbloodpressureparametersobtainedby 24-hourABMPandcerebralWMLs. 1.Introduction thatappropriateantihypertensivetreatmentcoulde?ciently preventthedevelopmentofWMLandslowtheirprogression [5,6]. Onageing,anonmodi?ablecardiovascularriskfactor,hyper- tensionbecomesthemostimportantmodi?ableriskfactor fordevelopingcerebrovasculardisease,includingstroke,ce- rebral small vessel disease (lacunar infarcts, white matter lesions,microbleeds),andcognitiveimpairmentorvascular dementia.
