Understandingthegreatvesselsoftheaorticarchandtheirvariationsisimportantforboththeendovascularinterventionistand thediagnosticradiologist.Anunderstandingofthevariabilityofthevertebralarteryremainsmostimportantinangiographyand surgicalprocedureswhereanincompleteknowledgeofanatomycanleadtoseriousimplications.Inthepresentcase,abilateral variationintheoriginandcourseofvertebralarterywasobserved.Theleftvertebralarterytookoriginfromthearchofaortaand enteredtheforamentransversariumofthefourthcervicalvertebra.Therightvertebralarterytookoriginfromtherightsubclavian arteryclosetoitsoriginandenteredtheforamentransversariumofthethirdcervicalvertebra.Theliteratureonthevariationsof thearteryisstudiedanditsclinicalsigni?canceandontogenyisdiscussed. 1.Introduction Thevertebralarteryarisesfromthesuperioraspectofthe subclavianartery,passesthroughtheforaminaofallcervical transverse processes except the seventh, curves medially behindthelateralmassofatlas,andthenentersthecranium via the foramen magnum. At the lower pontine border, it joinsitsfellowtoformthebasilarartery.Occasionally,itmay entertheboneat?fth,fourth,orseventhcervicaltransverse foramen[6].
