Purpose.Bonelossisacommonphenomenonfollowingallogeneichaematopoieticstemcelltransplantation(allo-HSCT).The studyaimedontoleranceande?cacyofzoledronicacid(ZA)inpatientsafterallo-HSCT.Methods.40patients’withosteoporosis orosteopeniawererecruitedonthisphaseIIstudy.ZAwasgivenatadoseof4mgIVevery3monthsfor2years(yrs).BMDwas determinedbydual-energyX-rayabsorptiometry(LSlumbarspine,FHfemurhip).Patientswereevaluatedfordeoxypyridinoline (Dpd)andcalciumexcretionbylongitudinalmeasurements.Results.36patientswhohadreceivedatleast3dosesofZAwere evaluable.26patientshadatleasttwoBMDmeasurementssincebaseline(BMDgroup).Amongthesepatients,BMDincreased from0.97± 0.15to1.10± 0.18g/cm 2 (LSbaseline—2yrs,Δ+11.6± 6.0%,P0.001)andfrom0.82± 0.10to0.91± 0.10g/cm2 (FHbaseline—2yrs,Δ+7.5± 7.0%,P 0.001).FactorsassociatedwithanincreaseinBMDwereyoungerage,femaledonor sex,andimmunosuppressionwithCSA/MTX.Conclusion.ZAwasgenerallywelltolerated;itincreasesBMDandreducesDpd excretionsigni?cantlyinpatientswithboneloss
