Thispaperfocusesonrecentexperienceinsettingupanendoscopyunitinalargeregionalhospital.Themixofendoscopyin threesmallerhospitals,drainingintothelargehospitalendoscopyunit,hasenabledtheauthorstocommentonpracticaland achievablestepstowardscreatingbestpracticeendoscopyintheregionalsetting.Thechallengesofusingwhatisavailablefroman infrastructuralequipmentandpersonnelsettingarediscussed.Inafastmoving?eldsuchasendoscopy,newtechniqueshavean importantroletoplay,andsomeareindeedcoste?ectiveandhavebeenshowntoimprovepatientcare.Someofthenewtechniques and technologies are easily applicable to smaller endoscopy units and can be easily integrated into the practice of working endoscopists.Coste?ectivenessandpatientcareshouldalwaysbethe?nalarbiterofwhatisessential,asopposedtowhatis nicetohave.Closecooperationbetweenreferralandperipheralcentersshouldalsoguidethesedecisions. 1.Introduction conducts outreach endoscopy in 3 peripheral hospitals servingsmalltownsoflessthan30000people,upto70km fromthebasehospital.Thebasehospitalhas400bedsand is a community hospital. We have very recently extended our endoscopy unit, transforming it from a single theatre complex-basedroom,intoastandal
